Job description

Medical Scribe — Physician Documentation Specialist
Skilled Wound Care | Remote — Philippines Based

About the Role

Skilled Wound Care is seeking a detail-oriented and English-proficient Medical Scribe to support our wound care physicians across the United States. You will work alongside our physicians — transcribing clinical notes, documenting patient encounters, and ensuring accurate records in our Electronic Health Record system.

This is a fully remote position based in the Philippines. If you are a clear communicator, a fast and accurate typist, and have a passion for healthcare — we want to hear from you.

What You Will Do

  • Transcribe clinical notes
  • Document wound assessments, treatment plans, procedure notes, and physician orders accurately
  • Enter all documentation into designated EHR platforms promptly
  • Ensure notes are complete and submitted for physician review within required timeframes
  • Upload medical records and supporting documents to payer portals as needed
  • Communicate clearly and professionally with physicians and clinical staff in English at all times
  • Flag incomplete or missing documentation to the physician promptly
  • Participate in regular check-in calls with your physician and operations team

What We Are Looking For

  • This role requires strong English communication skills — both spoken and written — as you will be working closely with US-based physicians and healthcare teams
  • Minimum 1 year of experience in medical scribing, transcription, or healthcare administrative support
  • Basic understanding of medical terminology and clinical documentation
  • Familiarity with wound care terminology preferred — wound staging, debridement, NPWT
  • Experience with EHR platforms — PointClickCare, Epic, Cerner, or similar
  • Strong attention to detail and ability to meet documentation deadlines
  • Reliable high-speed internet
  • Dedicated quiet home workspace with noise-cancelling headset
  • Self-motivated and dependable in a fully remote environment

Work Schedule

  • Fully remote — work from home in the Philippines
  • Schedule aligned with US Central or Eastern Time — Philippine evening and overnight hours
  • Monday through Friday — full time
  • Occasional Saturday availability may be required

HIPAA and Compliance

This role involves access to Protected Health Information (PHI). All team members must complete HIPAA training, sign a Business Associate Agreement, use SWC-approved encrypted platforms at all times, and maintain a secure private workspace during working hours.
