  • Strong planning, organizational, and follow-up skills; team player; ability to travel between Staten Island and Brooklyn as needed

Requirements:

  • Monitor safety and well-being through casework contacts and home visits; co-develop, coordinate, and monitor the service plan in alignment with the family’s needs; ensure medical, mental health, academic, housing, and career goals are achieved
  • Document all activities as progress notes in the system within program-defined timeframes; participate in Family Team Conference and complete FASP by deadlines; attend Family Court hearings and provide testimony; complete court and Permanency Hearing reports
  • Maintain frequent, positive, and consistent contact between the child and parents through in-person family time visits and phone/video contact; proactively problem-solve barriers to reunification and permanency options
  • Participate in treatment planning efforts and Trauma System Therapy meetings; engage in job development and professional development activities

Job Summary:

The Case Planner is responsible for ensuring the safety, permanency and well-being for children placed into foster care with Seamen’s Society. Working alongside the family in partnership, the Case Planner is the lead on all service planning activities. As a key member of the service planning team, the Case Planner facilitates the family’s plan through direct intervention and coordination with other providers and team members.   


Duties and Responsibilities:

  • Monitors safety and well being directly through casework contacts and home visits with the child, parents, and foster parents.
  • Co-develops, coordinates, and monitors the service plan in alignment with the family’s needs.
  • In collaboration with the interdisciplinary team ensures that child’s medical, mental health, academic, and where applicable housing and career goals are being achieved.
  • Documents all activities as progress notes in Connections within program defined timeframes. 
  • Acts as a key participant in the Family Team Conference and is responsible for completing the FASP in accordance with program deadlines.
  • Attends Family Court hearings for all cases assigned as well as provides written and verbal testimony as to the family’s progress in meeting their service planning goals.
  • Completes court reports and Permanency Hearing reports in accordance with program guidelines.
  • Ensures frequent, positive, and consistent contact between the child and their parents through the facilitation of in person family time visitations as well as phone and video contact. 
  • Proactively problem solves any barriers towards reunification and other permanency options
  • Is there for the family providing meaningful and responsive actions in times of need
  • Actively participates in treatment planning efforts as well as Trauma System Therapy team meetings as appropriate.
  • Participates in job development and professional development activities



Requirements

Skills, Knowledge, and Abilities:

  • Microsoft 365 proficiency and knowledge of Connections
  • Team player with strong planning, organizational and follow up skills
  • Ability to perform the essential functions of the job with or without a reasonable accommodation
  • Ability to handle and resolve recurring problems


Qualifications/Requirements:

  • Bachelor’s degree and driver’s license. Prior experience preferred. 
  • Our Agency operates in 2 locations, Staten Island and Brooklyn. The ability to travel between sites may be necessary.
  • Due to the nature of the work environment work hours will be based on needs of program and may vary.
  • Performs other duties/responsibilities as assigned within the scope of position
