Job description

We are sharing a specialised part-time consulting opportunity for currently practising Hospitalist and Internal Medicine Physicians with strong inpatient clinical documentation experience and advanced proficiency in English plus at least one additional language.

This role focuses on reviewing, annotating, and evaluating inpatient clinical documentation for accuracy, completeness, and clinical soundness. Selected physicians will apply their current hospital medicine experience to assess H&Ps, progress notes, discharge summaries, and encounter data while providing structured clinical feedback.

Key Responsibilities

Inpatient Clinical Documentation Review

  • Review hospital documentation including H&Ps, daily progress notes, and discharge summaries
  • Assess documentation for clinical accuracy, completeness, and appropriateness
  • Evaluate whether notes reflect what a practising hospitalist would reasonably document
  • Identify missing, unsupported, or clinically inconsistent information
  • Apply current inpatient clinical judgement across assigned cases

Clinical Note Evaluation

  • Review generated inpatient notes and summaries against source encounter information
  • Identify hallucinated, inaccurate, incomplete, or clinically inappropriate content
  • Assess whether documentation accurately represents the patient's hospital course
  • Evaluate clinical reasoning and documentation quality
  • Flag issues that could affect clinical usefulness or patient safety

Annotation & Structured Review

  • Annotate inpatient encounter data according to detailed project guidelines
  • Apply annotation standards consistently across cases
  • Categorise documentation issues and clinical discrepancies
  • Provide structured written feedback on identified problems
  • Maintain precision across repeated clinical review tasks

Hospital Medicine Expertise

  • Apply hands-on experience from hospital medicine or internal medicine
  • Evaluate inpatient presentations, clinical progression, and documentation workflows
  • Review assessments and documentation within realistic hospital contexts
  • Use current clinical experience to identify unrealistic or incomplete documentation
  • Apply practical judgement grounded in active inpatient practice

Multilingual Clinical Evaluation

  • Review or assess clinical material requiring advanced proficiency in a language other than English
  • Evaluate linguistic accuracy alongside clinical meaning where relevant
  • Apply professional-level speaking, listening, and written comprehension
  • Identify discrepancies that may alter clinical interpretation
  • Support evaluation of multilingual healthcare documentation

Guideline Development & Refinement

  • Provide expert input on annotation guidelines and inpatient documentation standards
  • Identify ambiguous instructions or edge cases
  • Raise clarifying questions where project guidance requires refinement
  • Help define clinically appropriate treatment of unusual documentation scenarios
  • Contribute practical hospitalist insight to quality standards

Clinical Quality Feedback

  • Provide concise, actionable written feedback
  • Explain why particular documentation is clinically accurate or problematic
  • Distinguish material clinical issues from minor documentation differences
  • Support consistent evaluation across large sets of clinical material
  • Communicate findings clearly to technical and product stakeholders

Ideal Profile

  • MD with a background in hospital medicine or internal medicine
  • 2+ years of post-residency experience
  • Currently practising in an inpatient setting
  • US-based
  • Hands-on experience writing and reading at least one of the following:
    • History & Physicals (H&Ps)
    • Progress notes
    • Discharge summaries
  • Advanced proficiency in English
  • C1-level or higher speaking, listening, and writing proficiency in at least one of the following:
    • Spanish
    • Mandarin Chinese
    • Cantonese Chinese
    • Tagalog
    • Russian
    • Haitian Creole
    • Korean
    • Arabic
    • Portuguese
    • Polish
    • Japanese
    • Turkish
    • Italian
    • Ukrainian
    • Romanian
    • Finnish
    • French

  • Minimum availability of 10 hours per week
  • 5+ years of post-residency experience is preferred
  • Familiarity with Epic is advantageous
  • Previous clinical annotation or model-evaluation experience is preferred
  • Experience documenting patient care in a language other than English is advantageous

Engagement Details

  • Part-time independent contractor engagement
  • Fully remote within the United States
  • Minimum commitment of 10 hours per week
  • Flexible scheduling designed to accommodate an active clinical practice
  • Compensation: Up to $160/hour
  • Work focuses on inpatient clinical documentation review, annotation, multilingual clinical evaluation, and structured quality feedback
  • Projects may be extended, shortened, or concluded based on project needs and performance
  • Work must be completed without using confidential or proprietary information belonging to any employer, client, healthcare organisation, institution, patient, or other third party
  • H1-B and STEM OPT support is unavailable for this engagement
