JOB TITLE: Clinical Pharmacist

REPORTING TO: Lead Pharmacist

LOCATION: Remote

SALARY: Up to £50,000 per annum, pro rata

Contract: 12 month fixed term contract, part time, 21 hours per week

ROLE OVERVIEW:

The post-holder supports the safe and effective delivery of medicines management across Medefer's NHS and private patients. Working within Medefer's clinical prescribing pathway, the post-holder will review consultant prescribing recommendations, undertake full clinical pharmacy checks, counsel patients by telephone, and issue prescriptions using remote prescribing software.

The role operates across multiple Integrated Care Boards (ICBs) spanning gastroenterology, rheumatology, endocrinology, and a growing range of specialties. The post-holder will work autonomously under the oversight of the Lead Pharmacist, exercising sound clinical judgement and maintaining clear communication with consultants, GPs and patients.


KEY RESPONSIBILITIES:

• Review consultant prescribing recommendations and undertake a full clinical pharmacy check prior to issuing a prescription, including allergies, dose, frequency, formulation, interactions, clinical appropriateness, and local ICB formulary compliance.

• Issue prescriptions for NHS and private patients using remote prescribing software, in line with Independent Prescriber registration and Medefer's prescribing policies and SOPs.

• Apply knowledge of local ICB formularies, Medefer operates across multiple ICBs each with distinct formulary requirements and prescribing restrictions.

• Provide detailed feedback to consultants where a prescription requires amendment; utilise clinical competence to make safe changes within scope where appropriate.

• Counsel patients by telephone on their medicines including indications, side effects, monitoring requirements, and when to seek further advice.

• Liaise with consultants and GPs where prescriptions require clarification, amendment, or escalation.

• Manage prescribing of specialist and high-risk medicines in accordance with relevant shared care arrangements and governance requirements.

• Identify and escalate medicines safety concerns, interactions, and contraindications promptly.

• Maintain accurate records of all clinical interventions and patient contacts.

• Support the Lead Pharmacist with medicines governance activities, including audit, quality improvement and pathway development.


QUALIFICATIONS, EXPERIENCE, SKILLS AND COMPETENCIES:

Essential

• MPharm degree and current GPhC registration.

• Qualified as an Independent Prescriber, this is a requirement for the role and must be active at the point of engagement.

• Experience in a secondary care, outpatient or hospital pharmacy setting.

• Membership with PDA or equivalent professional indemnity in place.

• Minimum 2 years post-registration clinical experience with active Independent Prescriber practice.

• Proficient in remote clinical and prescribing systems; able to work effectively in a fully remote environment.

• Strong verbal and written communication skills, with the ability to convey clinical information clearly to patients, GPs and consultants.

Desirable

• Experience working in a remote or virtual clinical environment.

• Postgraduate certificate or diploma in pharmacy practice (PGCert or PGDip).

• Familiarity with local ICB formularies and shared care protocols.

• Involvement in medicines governance, SOP development or clinical audit.


REQUIRED WORKING ARRANGEMENTS:

  • 21 hours per week 9am to 5pm

  • A private and confidential space to work in at home when working remotely.
