Job description

This is a remote position.

As a Remote Pharmacist Coordinator, you will be responsible for developing comprehensive medication management plans, coordinating patient care services, and ensuring that individuals receive optimal therapeutic outcomes. In this role, you will consult with healthcare providers, evaluate patient clinical progress, and assist with the complexities of insurance claims.

Responsibilities
  • Act as an empathetic resource for patients, providing clinical reassurance, emotional support, and encouragement regarding their medication journeys and health goals.
  • Maintain precise and accurate records of patient pharmaceutical care, medication therapy reviews, and provider schedules.
  • Guide patients through the telepsychiatry process, assisting them with platform technologies and addressing concerns about remote clinical consultations.
  • Collaborate directly with patients to identify unique therapeutic needs and develop personalized medication care plans that align with their specific goals and preferences.
  • Serve as a dedicated advocate for patients' rights, ensuring they have access to necessary medications and appropriate clinical services.
  • Conduct regular clinical check-ins to evaluate, document, and report on patient progress and adherence to prescribed therapies.
  • Facilitate communication between patients and the healthcare team, ensuring timely scheduling of medication reviews and follow-up consultations.
  • Address patient inquiries regarding prescription billing, insurance coverage, and preparation for clinical appointments.
  • Proactively manage the cancellation or rescheduling of appointments to maintain clinic efficiency.
  • Keep patients informed of any changes to their schedule, ensuring they feel supported and well-informed throughout their treatment.
  • Provide accurate, up-to-date information regarding mental health conditions, pharmacotherapy options, and available community resources.


Requirements

  • Bachelor’s degree in Pharmacy, Psychology, Nursing, Healthcare Administration, or a closely related clinical field.
  • 5+ years of experience in a patient coordinator, clinical pharmacist, or clinical associate role, preferably within a remote healthcare environment.
  • Excellent verbal and written communication skills (English-C1) to effectively relay clinical updates to patients and medical staff.
  • Strong organizational skills and attention to detail for the thorough documentation of clinical interactions.
  • Knowledge of mental health conditions, psychiatric treatment modalities, and community health resources is highly desirable.
  • Proficiency with telehealth platforms and remote monitoring technologies is a plus.
  • Ability to work independently, take initiative, and collaborate effectively within a multidisciplinary clinical team.
  • Must be willing to work 9 AM to 6 PM EST, on a schedule of either Monday–Friday or Sunday–Thursday, depending on clinic needs.
  • A strict commitment to maintaining patient confidentiality (HIPAA) and adhering to the highest ethical standards of pharmacy practice.
