If yes, join the great team of people at Koinonia Family Services in Los Angeles (Gardena) working in our enhanced care management (ECM) program as a part-time ECM Care Coordinator. As a Care Coordinator, your role involves connecting individuals to services and resources best aligned to meet their unique needs in the greater Los Angeles area. You'll undertake assessments, develop individualized care plans, oversee service coordination and referrals, and provide education to equip individuals with the tools to make informed, healthy lifestyle decisions. Bilingual skills in English and Spanish are strongly preferred.
Enhanced Care Management is a statewide Medi-Cal benefit available to individuals with complex needs. The Care Coordinator (aka Lead Care Manager) will lead the assessment, care plan development and ongoing care management of eligible participants under the supportive guidance of an ECM Supervisor. Our program specifically targets foster youth and other at-risk youth populations, but may also include their family members or other adults. The Care Coordinator utilizes motivational interviewing to engage individuals to support them in better managing their chronic conditions. The Care Coordinator actively engages individuals to remove barriers to health, social services, and behavioral health care.

This is a part-time role, initially offering 20β25 hours per week. Because this is a caseload-driven position, scheduled hours may increase or decrease over time based on client assignments and business needs. Applicants should be available to work at least 20 hours per week, although weekly schedules may vary (no minimum hour guarantee). As caseloads grow, additional hours may become available; if caseloads decrease, scheduled hours may be adjusted accordingly. Shifts are generally scheduled during normal business hours and may extend into the early evening based on member needs. Some scheduling flexibility is available to accommodate your personal schedule, provided client and business needs continue to be met.

Qualifications:
We are seeking candidates who speak Spanish (preferred) and have at least two years of related work experience in social work, case management, community engagement or customer service. High school graduate or GED required. Prior experience as a community health worker, peer support specialist, medical assistant or similar role is a plus.

This position requires a valid driverβs license, auto insurance and reliable transportation as driving is an essential job function. ECM Care Coordinators are responsible for tasks such as visiting client homes, attending in-person meetings, and engaging with community partners, often in locations not easily accessible by alternative transportation methods. The nature of these responsibilities necessitates the ability to drive to ensure timely, reliable, confidential and effective support for the youth and families we serve.
