About the Role

As a Nurse Advisor on PHM’s Health Advisory team, you’ll manage a portfolio of clients as they navigate the healthcare system and help them obtain high-quality care. Drawing on your clinical expertise and a compassionate, consultative approach, you’ll educate clients, advocate on their behalf, coordinate care, and help them make informed healthcare decisions with confidence.

What You’ll Accomplish

  • Build trusted relationships with clients and serve as a reliable guide throughout their healthcare journey.
  • Help clients access appropriate providers, facilities, and services while reducing the complexity of coordinating care.
  • Ensure smooth transitions and consistent communication across PHM’s clinical and operational teams.
  • Deliver a compassionate, high-quality client experience across a portfolio of low- to moderate-acuity cases.

How You’ll Spend Your Days

  • Manage a caseload of clients delivering responsive transaction based clinical support by providing education, advocacy, care coordination, and ongoing support.
  • Conduct a clinical intake via email or phone call to assess the client’s needs and determine the appropriate step forward in their healthcare journey.
  • Collaborate closely with client services coordinators, medical records, and provide network team to holistically manage cases to provide the appropriate provider recommendations, coordinate appointments, assist with medical record review and support, and provide clinical guidance when necessary
  • Provide cross collaboration support to our triage team who handles inbounds via email and phone calls for clients with active needs or who are interested in our services
  • Communicate clearly and compassionately with clients, caregivers, providers, and internal partners.

What You Bring to the Table

  • An active Registered Nurse license in your state of residence and at least five years of healthcare experience.
  • Experience in case management, care coordination, patient navigation, or a related clinical environment.
  • The ability to consistently work Monday through Friday from 8:30 a.m. to 5:00 p.m. Pacific or Mountain Time.
  • Experience working remotely
  • Confidence, empathy, critical thinking skills and sound clinical judgment when supporting clients and their families.
  • Strong written and verbal communication skills, with the ability to translate complex clinical information for both clinical and non-clinical audiences.
  • A self-directed, collaborative approach, supported by strong organization, time management, and comfort learning new technologies and systems.

Compensation

The target base salary for this role is $85,000–$100,000 annually.

Base salary is one component of PHM’s total compensation package, which also includes health, dental, and vision benefits; an annual cash incentive program; a 401(k) with company match; flexible paid time off; PHM for PHM, which provides our services to employees and their eligible dependents; and additional benefits.

Individual compensation may vary based on factors including experience, skills, location, market conditions, internal equity, and other relevant business considerations.

Location and Work Schedule

This is a remote role requiring that you live in and physically perform all work in the United States.

Candidates may be located anywhere in the United States; however, the ability to consistently work 8:30 a.m. to 5:00 p.m. Pacific or Mountain Time is required. Priority may be given to candidates who reside in the Mountain or Pacific time zones.
