Cardiovascular disease remains the leading cause of death in America. At Chamber, we’re rebuilding the system for cardiology, creating a world where outcomes, not volume, define success. We partner with independent cardiologists to help them lead population health efforts in their communities, equipping them with technology, data, and operational tools that turn complex insights into better care for patients.
Our model blends clinical expertise, thoughtful design, and a modern operating platform that supports physicians, patients, and payers alike. We believe innovation and empathy go hand in hand, and by combining cutting-edge AI tools with a relentless focus on human care, we can transform heart health at scale.
We’re looking for a Director, ACO Operations to launch and scale Chamber’s Accountable Care Organization, going live in January 2027. You’ll launch the ACO, run its operations, and prove that a specialty-focused model can outperform in cardiology.
The work spans governance and compliance, payment operations, and network performance — and rewards someone who moves fluidly between regulatory detail, execution, and strategic judgment. You’ll partner closely with Finance, Provider Network, Clinical, and Product to make it work.
This is a role with real ownership and real consequence. You won’t administer a fixed playbook — you’ll write one, and define what a specialty ACO can be. We’re looking for an operator who is excited for expanded accountability, and an opportunity to leave their mark on a brand new organization.
• Launch Chamber’s ACO and own its performance, holding the operating model, workflows, and controls to a high standard as the network scales.
• Prove the specialty-focused model in cardiology, using early results to shape how the network grows.
• Own the CMS submission and enrollment cycle end to end, building the calendar and controls that keep every filing accurate and on time.
• Serve as the operational and analytic engine of the ACO, exercising independent judgment on performance, compliance, and provider success.
• Establish and manage ACO governance — the committee structures, decision rights, and reporting cadences that keep the entity accountable and audit-ready.
• Own regulatory compliance across the program (e.g., MSSP, ACO REACH), monitoring requirements and adapting operations to policy changes.
• Serve as Chamber’s representative to CMS, federal agencies, and external partners on matters affecting ACO outcomes.
• Partner with Finance and Provider Network to design and run payment operations, translating value-based contracts into working financial workflows — attribution, reconciliation, and settlement.
• Track performance against financial and contractual metrics, surfacing risks and opportunities to leadership with clear, data-grounded recommendations.
• Partner with Clinical and Network teams to improve network performance against quality, cost, and utilization targets.
• Analyze performance data to develop strategic recommendations on ACO operations, quality improvement, and total cost of care.
• Master the ACO launch plan — operating model, compliance obligations, and open risks — and align leadership on the critical path to launch.
• Own the CMS submission and enrollment work for launch, standing up the processes and documentation that keep every filing accurate and on time.
• Partner with Finance and Network to ready payment operations, pressure-testing attribution, reconciliation, and provider payment before launch.
• 6–10+ years in healthcare operations, with meaningful time in value-based care, population health, or ACO/managed care.
• Deep knowledge of Medicare value-based programs (e.g., MSSP, ACO REACH, Medicare Advantage) and their CMS requirements.
• Track record of independently owning and scaling complex operational initiatives to measurable outcomes.
• Strong command of payment operations and value-based contract mechanics.
• Highly analytical, and comfortable in the data yourself — manipulating large files and datasets without waiting on others.
• Clear communicator who can distill complex program rules and payment mechanics into sharp takeaways for executive audiences.
• Entrepreneurial operator who does their best work amid ambiguity and is accountable for outcomes, not activity.
Our values guide how we lead, collaborate, and care:
• Low Ego: We stay grounded, curious, and open to feedback.
• Empathy: We build trust through compassion and thoughtful communication.
• Courage: We take action, think critically, and challenge ideas respectfully.
• Ownership: We follow through with integrity and hold ourselves to high standards.
• Grit: We push through ambiguity, move with urgency, and solve problems with horsepower and heart.
Remote, with a preference for candidates based in the Eastern or Central time zone. Periodic travel — expected roughly quarterly — to Chamber practices or offices may be required.
