Sentara Health is hiring a Full Time Care Coordinator for the South Central Richmond, VA area!

Hours: Full Time (40 hrs/wk)

Shift: Day (8am-5pm)

This position requires travel for in person face-to-face assessments in around the south Central Richmond- Dinwiddie, Petersburg, Prince George, SouthHill . Days without assessments scheduled employee can work remotely.

Applicants must be located in the south Central Richmond Area of VA

Education

  • Must possess a Bachelor's degree in Health & Human Services or one of the following related Fields:

    -Art Therapy
    -Behavioral Sciences
    -Child Development
    -Cognitive Sciences
    -Community Mental Health Counseling (MH, Vocational, Pastoral, etc.)
    -Counselor Education
    -Early Childhood Development
    -Educational Psychology
    -Gerontology
    -Healthcare Administration

    -Human Services
    -Marriage and Family Therapy
    -Music Therapy
    -Nursing
    -Pharmacy
    -Psychiatric Rehabilitation

Certification/Licensure

  • No specific certification or licensure requirements

Experience

  • 1 year of social work, care coordination, case management, behavioral health, healthcare, or community-based services experience (Required)

  • Experience supporting individuals with Intellectual and Developmental Disabilities (ID/DD) and waiver services (Preferred)

  • Experience working with individuals affected by substance use disorders and coordinating treatment or recovery support services (Preferred)

  • Experience advocating for patients, clients, or vulnerable populations and assisting with care transitions and service navigation (Preferred)
