Posting Salary Range: $56,270 - $69,898

Office Location: Remote within Trillium’s catchment area of North Carolina (See requirements section for included counties)

What We’re Looking For


Trillium Health Resources has a career opening for a Housing Development Coordinator to join our team. Where your efforts will focus on assisting individuals identified in the Transitions to Community Living (TCL) Special Needs Population. Your work will primarily aim to divert and transition members from adult care homes into permanent supportive housing within the community. As a key player in this initiative, you will maintain a comprehensive understanding of local, state, and federal laws, as well as initiatives related to supportive housing. Your responsibilities will include inventorying currently available resources and educating the community and providers about TCL. You will also engage in directed care coordination activities related to acquiring supportive housing, which encompasses treatment planning, assessment, monitoring, educating, and providing referrals to members who are in the process of transitioning to supportive housing. 


On a typical day, you might:


  • Assist members in locating appropriate housing based on program requirements, their preferences and availability.
  • Educate landlords/property managers/owners on TCL and the TCL Voucher subsidy.
  • Provide technical assistance to landlords regarding specific members.
  • Work collaboratively with members and guardians to assess members’ financial needs and assist members in developing a budget that will allow them to successfully live independently.
  • Coordinate directly with members, landlords, tenancy providers, internal TCL, Care Management, and Finance staff, and other supports to identify and address housing issues and barriers, as well as any other social determinants of health, as a means to maintain permanent supportive housing, avoid formal evictions, and procure alternative housing through TCL if warranted.

Qualifications


Required:

  • Bachelor's degree with at least two (2) years of experience working with individuals and the housing systems serving people with SMI/SPMI, obtaining and maintaining Permanent Supportive Housing (PSH).
  • Must have a valid driver’s license
  • Must reside within Trillium’s catchment area, which includes the following counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, Wilson, Carteret, Craven, Duplin, Jones, Lenior, Onslow, Pender, Sampson, Wayne, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, Scotland, Anson, Guilford, Montgomery, Randolph, and Richmond. 
  • Must be able to travel within catchment as required.


Preferred:

  • Individuals who have experience working with the TCL population and are certified peer specialists.
