The Opportunity

Our Amplar Preventative Health team helps Australians improve their health, prevent avoidable hospital admissions and achieve better long-term outcomes.

We have an exciting opportunity for a Cardiac Rehab Program Clinician to join our national Heart Health at Home program.

 

Using your cardiac rehabilitation expertise, you'll support participants recently discharged from hospital following a cardiac event. Managing your own caseload, you'll deliver evidence-based coaching, education and care planning via phone/video, helping participants build confidence, improve heart health and reduce the risk of future events.

Key responsibilities include:

  • Delivering virtual cardiac rehabilitation coaching and education.
  • Conducting assessments and developing personalised care plans.
  • Monitoring participant progress and managing clinical risks.
  • Supporting participants to engage with the Cardihab app to support their rehab
  • Communicating with GPs, specialists and other healthcare providers to coordinate care.

 

 

This is a part-time opportunity (0.6 FTE), initially offered as a 12-month fixed-term contract. The role is fully work-from-home and requires a private workspace and reliable high-speed internet connection. We'll provide the equipment, training and support you need to succeed.

 

 

About You

You have a natural ability to build rapport with a wide range of clients and healthcare professionals and effectively influence for success. Working in this highly autonomous position you will have an organised and flexible mindset that allows you to meet the unique needs of your clients. Additionally, you will also have the following skills/experience:

 

  • At least 5 years professional clinical experience as either a Registered Nurse, Exercise Physiologist or Physiotherapist with current registration with AHPRA or ESSA
  • 2+ years’ experience delivering cardiac rehabilitation (phase 2) within the past 5 years.
  • Previous health coaching experience and understanding of health behaviour change methodology.
  • Experience in acute and primary health care and care planning.
  • Solid understanding of cardiac rehab programs, cardiac disease, and evidence-based practice.
