Head of Integration Engineering

Roles & Responsibilities

  • Built production-grade integration systems at scale (REST, gRPC, webhooks, message buses like Kafka/RabbitMQ) with robust retry and failure handling.
  • Healthcare EHR experience or fast-learning capability with Epic, Athena, Cerner, their sandboxes, certification, and SMART on FHIR flows.
  • Hands-on coder with a player-coach leadership style, setting the bar on system design, code quality, and connector reliability.
  • Led engineering teams—hiring, mentoring, driving performance and a culture of reliability and ownership—in high-growth environments.

Requirements:

  • End-to-end ownership of Sully's EHR integration layer across Athena, Epic, Cerner, eClinicalWorks, AdvancedMD, Tebra, Elation, NextGen, and more.
  • Integration strategy and roadmap to unlock ARR, define a repeatable framework, and achieve the Q4 target of 5 production EHR connectors.
  • Reliability and observability: p95 deploy time, error budgets, on-call rotation, automated monitoring, and incident rate reduction.
  • FHIR, HL7, and custom partner API strategy to standardize how Sully communicates with diverse healthcare systems.

Job description

About Us
At Sully.ai, We’re Building the Most Impactful Healthcare Company on Earth

We believe that access to a great doctor is a basic human right. Today, that’s not a reality. Delays, misdiagnoses, administrative chaos, and burnout plague the system.

Our Mission
One Human, One Doctor.
We enable our customers to staff 30% of their workforce with AI by creating a shared agent architecture for scale and efficiency. All powered by our own patented, world-class models and deployed in real-world care.

About the Role

Sully automates clinical documentation at the point of care. Real-time ambient listening, structured medical notes, and agentic AI workflows that live inside the EHR. We're live in 270+ clinics, have processed 2M+ real physician-patient encounters, and our integrations are the bridge between our AI and the systems clinicians actually use every day.

We're hiring a hands on Head of Integration Engineering to own Sully's EHR and partner integration layer end-to-end. This is the most commercially critical engineering domain at the company. We have product market fit and pure demand signal waiting to be unlocked.

Every EHR connector you build or improve directly compounds revenue, retention, and clinical impact. You'll build and scale the team, define the reliability bar, and make integrations a competitive moat, not a source of incidents.

───

What You'll Own

End-to-end ownership of Sully's EHR integration layer: Athena, Epic, Cerner, eClinicalWorks, AdvancedMD, Tebra, Elation, NextGen, and more
Integration strategy and roadmap: prioritizing which connectors unlock the most ARR, defining a repeatable framework, and hitting the Q4 target of 5 production EHR connectors
Reliability and observability: p95 deploy time, error budgets, on-call rotation, automated monitoring, incident rate reduction
FHIR, HL7, and custom partner API strategy: unifying how Sully speaks the language of every healthcare system
SDK and partner onboarding: a clean, documented path for new EHR and API partners to integrate with Sully
Integration security and compliance: HIPAA, SOC 2, threat modeling, security reviews on every connector
Team building: hiring, mentoring, and scaling an integrations and reliability engineering team

───

What We're Looking For

You've built integration systems that actually work in production at scale: REST, gRPC, webhooks, message buses (Kafka, RabbitMQ), API gateway patterns. You've dealt with flaky third-party APIs, retry storms, and webhook delivery failures — and you've built systems that handle them gracefully.
Healthcare EHR experience is a strong signal: You know what it means to integrate with Epic, Athena, or Cerner, the sandboxes, the certification processes, the SMART on FHIR flows, the quirks. If you don't yet, you learn fast and you've integrated complex enterprise systems before.
You still write code: Player-coach. You set the technical bar by example on system design, code quality, and connector reliability.
You've led engineering teams: hired the best, driven performance, made people better, built a culture of reliability and ownership.
Security and compliance aren't afterthoughts: You've done HIPAA reviews, threat modeling, and SOC 2 prep. You know what data flows need BAAs and why.
You've operated in high-intensity environments: Seed through Series A/B, early-stage, or high-growth. You know how to prioritize ruthlessly when the integration queue is longer than the team.
Cloud and CI/CD fluency: GCP, AWS, or Azure; Docker, Kubernetes, GitHub Actions. You can ship and operate connectors in production.

───

First 90 Days

Audit all current integrations: reliability, error rates, security posture, and coverage gaps. Publish SLAs and on-call runbooks
Define and ship the 12-month integration roadmap: which EHRs unlock the most ARR, which partners are queued, what the repeatable connector framework looks like

Deliver one net-new EHR connector to production or pilot: establish the template for the Quarter target of 5 integrations
Establish gold-standard CI/CD and observability: p95 deploy time under 15 minutes, error budget policy, on-call rotation
Reduce integration incident rate and MTTR by 30%: through automated monitoring, alerting, and self-healing
• Kick off hiring for two key integration engineering roles

───

Why Sully

Real PMF: 270+ clinics live, 2M+ encounters processed, integrations are already working and already driving revenue
The commercial leverage is direct and measurable: every EHR connector you build unlocks a segment of the market.
The technical challenges are real: flaky EHR APIs, FHIR compliance, multi-tenant webhook delivery, real-time data sync at scale. This is not a maintenance role.
You're replacing a genuinely broken workflow (manual documentation = physician burnout), not adding an AI feature
Small team, outsized surface area, high ownership: If you want to own the integration strategy of the company defining how AI works in healthcare, this is it
