About the Role

As a Care Advocate I at Color, you will play a critical role in helping patients navigate complex healthcare journeys, ensuring they receive the care, guidance, and support they need.

You will serve as a primary point of contact and trusted partner for patients navigating cancer screening, abnormal results, elevated cancer risk, active cancer treatment, and survivorship. You will build meaningful relationships with patients, understand their individual needs, and help them overcome barriers to accessing high-quality care.

This role combines personalized patient advocacy with hands-on healthcare navigation and care coordination. You will help patients identify appropriate providers and facilities, coordinate referrals and appointments, navigate insurance and benefits, and stay on track with recommended care. You’ll also provide ongoing encouragement, education, and support to help patients feel informed, empowered, and confident in their next steps.

As part of a growing health technology company, you’ll need to be resourceful, tech-savvy, and adaptable. We’re looking for someone who can balance compassionate, high-touch patient care with efficiency, strong organizational skills, and a continuous improvement mindset.

How You'll Contribute:

  • Build meaningful patient relationships through empathetic communication, personalized guidance, and proactive engagement.

  • Help patients overcome barriers to care, make informed decisions, and stay motivated and on track with recommended care.

Healthcare Navigation & Care Coordination

  • Coordinate all aspects of patient care, including identifying high-quality providers and facilities, managing referrals, scheduling appointments, and facilitating follow-ups.

  • Proactively track patient progress, resolve barriers or delays, and ensure timely follow-through on care needs.

  • Collaborate with Color’s clinical teams and external providers to coordinate care, communicate updates, and escalate concerns appropriately.

Insurance & Benefits Navigation

  • Verify insurance eligibility, benefits, and coverage, helping patients understand their options and potential costs.

  • Coordinate prior authorizations, referrals, and financial assistance while troubleshooting insurance or administrative barriers to care.

Operational Excellence & Continuous Improvement

  • Manage a portfolio of patients and care coordination tasks, balancing high-quality patient support with efficiency, productivity, and timely follow-through.

  • Maintain accurate patient records, documentation, and task tracking while ensuring patient privacy and confidentiality.

  • Leverage technology and digital tools to streamline workflows, improve efficiency, and enhance the patient experience.

  • Adapt to evolving processes, tools, and business priorities, taking on new responsibilities and identifying opportunities for continuous improvement.

  • Experience in a patient-facing healthcare role, including referrals, provider networks, insurance benefits, and medical terminology.

  • Excellent organizational skills and attention to detail, with the ability to manage multiple patients, competing priorities, and timely follow-ups.

  • Strong problem-solving skills and resourcefulness, with the ability to independently navigate challenges, remove barriers, and take ownership of patient needs.

  • Tech-savviness and adaptability, with the ability to quickly learn new tools, embrace change, and thrive in a fast-paced startup environment.

  • A proactive, results-oriented mindset, balancing compassionate patient care with efficiency, productivity, and continuous improvement.

Nice to Have:
