Job Summary/General Purpose

Serves as the primary liaison between PACS hospice providers and their contracted partner facilities, including skilled nursing, assisted living, and other long-term care settings. Manages hospice-facility service agreements, strengthens referral and care-coordination relationships, and helps ensure contractual and regulatory compliance. Works closely with hospice leadership, facility administrators, and PACS Services teams to support timely access to hospice care and sustainable census growth.

  • Coordinate the drafting, negotiation, execution, and renewal of hospice-facility service agreements in partnership with Legal and Compliance, and maintain an accurate contract tracker with key terms and renewal dates.
  • Monitor adherence to contract terms, including care coordination, communication expectations, room and board arrangements, and billing processes, and address gaps promptly.
  • Ensure agreements and facility interactions align with applicable federal and state hospice regulations, including CMS Conditions of Participation, and support audit readiness.
  • Resolve service, communication, and care-coordination issues between hospice and facility teams, escalating complex or high-risk matters to leadership as appropriate.
  • Educate facility staff on the hospice benefit, eligibility, and referral processes to support timely, appropriate admissions and a consistent patient and family experience.
  • Track and report on referral volume, census, and partner facility performance, identifying trends and opportunities to expand and strengthen hospice partnerships.
  • Support onboarding of new partner facilities and new hospice locations, including orientation, documentation, and implementation of agreed workflows.
  • Collaborate with PACS Services teams, including Legal, Compliance, Finance, and Revenue Cycle, to resolve contract, billing, and compliance questions.
