Job description

The High Priority Reentry Peer Support Specialist responsible for providing re-entering individuals with Serious Mental Illness (SMI) and/or Severe and Persistent Mental Illness (SPMI), who are on the Department of Adult Correction (DAC) prioritization list, with education and support throughout the transition process. 

This position is required to reside in North Carolina within 40 miles of one of the counties within the Alliance catchment areas (Mecklenburg, Wake, Durham, Orange, Johnston, Harnett, or Cumberland).

Responsibilities & Duties

Provide Care Team Support

  • Support members transitioning from Inpatient settings to the appropriate lower or lateral level of care
  • Provide subject matter expertise, within scope, regarding member’s physical and/or behavioral health to support the development and delivery of a whole person approach to Care Management 
  • Collaborate with other Alliance staff, behavioral health providers, primary care physicians, specialty care providers and other community partners and stakeholders to support members in their home communities

Support High Priority Reentry Liaison Transition Coordinator with Core Re-Entry Transitional Care Coordinator Functions

  • Meet with clients regularly to offer one-on-one support and counseling
  • Require travel to meet with members regularly to offer one-on-one support and counseling
  • Develop recovery plans, coping methods, and rehabilitation strategies with the client
  • Assist members with developing short-term goals, coping methods and rehabilitation strategies with members
  • Arrange medical appointments, therapy sessions, support groups, and other resources to aid the client
  • Report any unusual behavior, conflicts, or recovery deviations to the relevant professionals
  • Report any challenges, conflicts, or barriers to the High Priority Reentry Liaison Transition Coordinator 
  • Liaison with family members and medical professionals regularly
  • Work with potential members family and medical professionals to address members needs and goals
  • Coordinate the client's logistics such as medical appointments and therapy sessions, among others
  • Address a member’s barriers including, not limited to, medical appointments, treatments, and other important appointments for services

Travel

  • Travel between Alliance offices, attending meetings on behalf of Alliance, participating in Alliance sponsored events, etc. may be required
  • Travel to meet with members, providers, stakeholders, attend court hearings etc. is required

Minimum Requirements

Education & Experience

Required:

High School Diploma with two (2) years of experience working with people with SMI/SPMI and their families or guardians. Lived experience with mental health conditions, substance abuse or trauma is required. 

NC Certified Peer Support Specialist Program certification is required.

Preferred:

Experience providing peer supports to individuals in and out of the Justice System. 

Knowledge, Skills, & Abilities

  • Computer proficiency in Microsoft Excel, Word and Outlook.
  • Ability to utilize computer equipment and web-based software to conduct work.
  • Ability to interact with various office staff as needed to support necessary workflows.
  • Ability to interact with healthcare professionals, patients, their families and other supports.
  • Ability to communicate effectively to individuals and groups through spoken, written and electronic media.
  • Ability to attend to detail, effectively prioritize and execute tasks in a timely manner.
  • Ability to work independently without a high degree of supervision.

Employment for this position is contingent upon a satisfactory background and MVR (Motor Vehicle Registration) check, which will be performed after acceptance of an offer of employment and prior to the employee's start date.

Pay Range

$26.27-$34.14

Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity
