Job description

Overview:

The VBC Care Coordinator partners with our Value-Based Care Clinical team to drive improved outcomes for chronic kidney disease and end-stage kidney disease patients. Acts as the first point of patient contact for incoming or outbound calls. Supports gathering and sharing of key documentation (e.g., discharge summaries) to drive care. Manages workflow in our care management platform.  

Responsibilities:

Essential Duties and Responsibilities include the following.  Other duties and tasks may be assigned. 

  • Make outbound or receive inbound calls from patients to schedule appointments and/or address program needs.  

  • Conduct health-related patient questionnaires, as needed. 

  • Documents all activities for quality and metrics reporting through the care management platform.  

  • Gathers and stores critical patient information to drive nurse and other care team member workflow.  

  • May coordinate patient transportation. 

  • Participates in continuous quality improvement initiatives to ensure department and company goals are met and exceeded. 

  • Maintains performance and quality standards based on established contact center metrics. 

  • Actively promotes GUEST customer service standards; develops effective relationships at all levels of the organization. 

  • Participates in team concepts and promotes a team effort; performs duties in accordance with company policies and procedures. 

  • Regular and reliable attendance is required for the job. 

  • Remote position, prefer candidates based in Dallas and San Antonio. 

Qualifications:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Requirements include:

  • High school graduate or equivalent. 
  • Previous customer service, contact center, healthcare experience (and in some markets- bilingual skills) are preferred. 
  • Proficient computer skills, including Microsoft Office (Word and Outlook) and ability to learn new systems
  • Must be bilingual in Spanish/English
