Nurse Consultant

Polaris Group, the SNF Clinical branch of Assembly Health, is seeking a dynamic, client-focused Nurse Consultant. We are looking for a clinically trained professional who stays current on federal and state regulations, Medicare requirements, MDS standards, reimbursement systems, and best practices affecting the long-term care industry. This individual will deliver high-quality consulting services to facilities nationwide and clearly communicate findings, risks, and practical recommendations to clients and leadership.

The ideal candidate is an experienced, detail-oriented clinician with strong assessment, analytical, and organizational skills. The candidate should be able to perform clinical and regulatory compliance audits, evaluate Medicare and MDS-related systems, and identify documentation and process concerns that may affect resident care, survey outcomes, compliance, and reimbursement. This individual will manage assigned facilities with an emphasis on service quality and cost-effectiveness and collaborate across teams to support client needs.

This is a full-time, exempt position reporting to the Consulting Department Manager. 

What You Will Do:

  • Provide consulting service delivery to our clients nationwide. Manage assigned client facilities in both service delivery and cost effectiveness.
  • Ensure timely delivery of consulting visits according to our contractual obligations. Ensure assigned profit and loss goals are met.
  • Submit internal reports/calendars per company guidelines. Submit client reports after completion of customer site visit per company guidelines.
  • Maintain effective and timely communication with leadership about customer issues.
  • Participate in team meetings, committees, and/or on conference calls.
  • Remain current on issues and regulations that affect the industry and long-term care field.
  • Always protect the confidentiality of residents and facility information.
  • Perform clinical and regulatory compliance audits, mock surveys, medical-record reviews, Medicare and MDS process evaluations, and other clinically focused consulting services.
  • Own other tasks and projects as required and assigned.

What it Takes to Join the Team:

  • Active Registered Nurse license
  • Ability to travel to client facilities weekly (75-80% travel requirement)
  • Five years of experience in long-term care required.
  • Previous experience as a Director of Nursing, state surveyor, or regional nurse consultant, preferred.
  • Experience in clinical compliance and regulatory oversight
  • Demonstrated knowledge of Medicare Part A skilled nursing facility coverage and documentation requirements, the MDS 3.0, the RAI Manual, PDPM, and the relationship between clinical documentation, coding, compliance, and reimbursement.
  • Knowledge of state and federal regulations related to long term care facilities.
  • Engage with a wide base of clients across the organization. Collaborate with interdepartmental team members to complete projects.
  • Superior written and verbal skills with the ability to adjust the message to fit the audience. Strong organizational and time management skills
  • Adaptable to change and willingness to learn different processes.

Salary Range$95,000—$105,000 USD

Compensation for this role is based on a variety of factors, including but not limited to, skills, experience, qualifications, location, and applicable employment laws. The expected salary range for this position reflects these considerations and may vary accordingly. In addition to base pay, eligible employees may have the opportunity to participate in company bonus programs. We also offer a comprehensive benefits package, including medical, dental, vision, 401(k), paid time off, and more.
