Job Summary
MGBfC strives to advance health equity, improve health outcomes, and promote the well-being of all patients by addressing health-related social needs and overcoming barriers to accessing care. The Community Health Worker Lead plays a critical role in achieving these goals by combining direct patient support with day-to-day leadership of the Community Health Worker team. Embedded within pediatric primary care, the Community Health Worker Lead collaborates closely with multidisciplinary care teams to support families with care coordination, assessment of unmet health-related social needs, and navigation to appropriate hospital and community resources. In addition to maintaining a patient-facing caseload, the Lead provides guidance, mentorship, training, and operational support to Community Health Workers, helping to ensure high-quality, equitable, and family-centered care. The Community Health Worker Lead serves as a resource to staff, promotes best practices in community-based care, assists with program coordination and workflow, and supports team operations, including work allocation and coverage in the absence of the Manager. While this is not a clinical role, it requires a strong understanding of or the ability to learn basic medical concepts, the ability to recognize when referral to a licensed clinician is appropriate, and a commitment to fostering the professional growth and success of the Community Health Worker team.Qualifications
