SUMMARY:

Reports to the Pharmacist-in-Charge. Under general supervision of a Pharmacy Supervisor is responsible for remote services which includes the interpretation, evaluation, preparation and dispensing of medications pursuant to a legitimate order in keeping with established policies, procedures, and standards of practice and scope of pharmacy services of all Brown University Health (BrownHealth )affiliates. Collaborates with multidisciplinary care providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost-effective medication use. Oversees and reviews the work of pharmacy technicians and ancillary support personnel as applicable.

RESPONSIBILITIES:

Establishes and maintains compliance with Brown University Health’s Flexible Work Arrangements Guidelines and Receives patient medication orders remotely when entered into the electronic health record (EHR) by a provider. Reviews and interprets each order to determine completeness, therapeutic appropriateness as it relates to the patient’s clinical status, concomitant drug therapy and diet, and with consideration of the aging processes and human developmental stages (neonate, child, adolescent, adult, geriatric). Contacts the provider, or covering provider, as necessary to initiate appropriate interventions to ensure therapeutic efficacy, minimize potential risks of drug therapy, and optimize cost effectiveness. Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications for the patient populations served. Provides drug information and consultation to prescribers, nurses, and other health care professionals regarding appropriate drug therapy, including drug interactions, side effects, incompatibilities, allergies, medication order discrepancies, drug stability problems, and drug administration. Proactively reviews and approves medication orders in the EHR including drug, dose, dose form, frequency, and duration of therapy. Ensures relevant patient specific parameters are present in the EHR and considers (i.e., diagnosis, known allergies, weight, age, etc.).Identifies, documents, resolves, monitors, and prevents drug therapy related problems, including contraindications, duplications of therapy, and drug interactions.Engages with other clinical pharmacists or clinical pharmacist specialists for complex clinical situations.Integrates patient-focused medication therapy management and pharmaceutical care in accordance with established clinical pathways or guidelines.Participates in prospective and retrospective pharmacokinetic dosing and monitoring of selected medications and recommends dose adjustments and laboratory monitoring as required.Ensures compliance with target drug programs, formulary management initiatives, medication management initiatives, and adverse drug event monitoring programs. Documents and reports all clinical interventions, medication errors, near misses, and adverse drug events in accordance with established protocols. Remotely reviews and approves formulations for sterile products requiring compounding or manipulation prior to dispensing. In accordance with established procedures, completes in-process and final checks for medications prepared by pharmacy technicians. Ensures accuracy, sterility, and stability of compounded products. Participates in continuous quality improvement activities and total quality management initiatives, including, but not limited to adverse drug reaction, pharmacist clinical intervention, and medication incident reporting programs. Maintains professional competence through participation in Pharmacy staff development and skills enhancement programs, including but not limited to ongoing independent study, and membership and participation in professional organizations. Performs other duties as assigned.

MINIMUM QUALIFICATIONS:

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.

  • Prior to start date, possesses current unrestricted licensure as a licensed pharmacist or a licensed 90-day temporary pharmacist, in the State of Rhode Island. Within 90 days of start date, must possess and maintain current unrestricted licensure as a licensed pharmacist in the State of Rhode Island. Prior to start date, possess current licensure as a registered pharmacist in the Commonwealth of Massachusetts.

  • Within 18 months of hire, must possess and maintain Board of Pharmacy Specialties certification in area of clinical pharmacy expertise [i.e., Pharmacotherapy (BCPS), Oncology (BCOP), Cardiology (BCCP), etc.

  • Successful completion of Post Graduate Year 1 (PGY-1) Residency from an accredited program and/or a minimum of 3 years clinical experience required. Clinical experience, knowledge, and development may be considered in lieu of formal training.

  • Effective interpersonal and communication skills, both written and verbal, to interact appropriately with colleagues and others. Effective organizational and presentation skills.

  • Knowledge of area/specialty specific technology, including, but not limited to, pharmacy information systems, automated dispensing technology, software, equipment, and automation.

  • Experience with MS Word, Excel, Outlook, and web browser.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

Employee will be responsible to establish a secure workspace environment to protect the confidentiality of sensitive information. Sensitive information includes confidential information, protected health information (PHI), and protected identifiable information (PII). Employee will be responsible for costs associated with setting up, furnishing, and managing their remote employee workspace and for costs of maintaining appropriate internet connectivity to access Brown University Health’s Information Services resources. Extended periods of time spent sitting. Requires the visual and manual dexterity to operate a computer. Occasional travel to Brown Health affiliates, as needed and/or in the event of Brown Health Information Services downtime.

Pay Range:

$50.49-$83.30
