Job description

We are sharing a specialised part-time consulting opportunity for experienced pharmacists with frequent professional use of UpToDate Lexidrug or Lexicomp and strong expertise in medication safety, drug interactions, dosing, contraindications, and patient-specific pharmacotherapy review.

This role focuses on evaluating complex medication-related analyses against real clinical workflows and reference standards. Selected pharmacists will assess whether drug interactions, dosing recommendations, contraindications, warnings, and therapeutic decisions accurately reflect the patient's complete medication list, current clinical data, and relevant pharmacy guidance.

Key Responsibilities

Medication Interaction Review

  • Evaluate complete medication lists for clinically significant drug interactions
  • Assess interaction severity, mechanism, and patient-specific relevance
  • Identify interactions or precautions that may be missed when reviewing medications individually
  • Distinguish between technically permitted combinations and therapeutically appropriate choices
  • Review findings using Lexidrug or Lexicomp interaction resources

Contraindications, Warnings & Clinical Appropriateness

  • Evaluate contraindications, boxed warnings, precautions, and Special Alerts within the context of individual patients
  • Assess whether recommendations appropriately account for comorbidities, treatment history, and other clinical factors
  • Reconcile newer Special Alerts with existing monograph guidance where relevant
  • Identify cases where a medication may pass basic safety checks while remaining a poor clinical choice

Dosing & Patient-Specific Review

  • Assess renal and hepatic dose adjustments using current patient information
  • Verify calculations involving laboratory values, body weight, and other relevant clinical parameters
  • Identify recommendations based on stale or incomplete patient data
  • Review paediatric, geriatric, pregnancy, lactation, or speciality dosing considerations where applicable
  • Determine whether recommendations reflect the complete clinical picture

Clinical Follow-Through & Quality Evaluation

  • Verify that appropriate clinical findings are carried through into orders, documentation, and care-team communication
  • Assess whether medication changes, prescriber notifications, or chart updates are appropriately documented
  • Identify incomplete workflows where a correct issue is recognised but appropriate action is missing
  • Provide structured written feedback explaining errors, omissions, and recommended corrections

Ideal Profile

Strong candidates may have:

  • At least 2 years of professional pharmacy experience
  • Frequent professional use of Lexidrug or Lexicomp, typically weekly or more, in a current or recent role
  • Strong practical knowledge of drug interactions, contraindications, warnings, precautions, and medication monographs
  • Experience applying renal and hepatic dosing guidance using current clinical data
  • Ability to evaluate full medication lists rather than isolated drug pairs
  • Strong clinical judgment and attention to medication-safety details
  • Experience determining whether recommendations are appropriate for a specific patient
  • Excellent written communication and structured analytical skills

Educational Background

  • A PharmD and active pharmacy licence are highly relevant
  • Equivalent verification-heavy clinical pharmacy experience may also be considered
  • Inpatient or hospital pharmacy experience is strongly preferred
  • Advanced training in pharmacotherapy, medication safety, clinical pharmacy, or a related area may strengthen an application

Nice to Have

  • Experience using Trissel's IV Compatibility
  • Strong knowledge of paediatric or weight-based dosing
  • Familiarity with Beers Criteria and geriatric medication precautions
  • Experience assessing pregnancy and lactation considerations
  • Knowledge of REMS programmes, controlled-substance restrictions, or restricted distribution
  • Experience comparing therapeutic alternatives for allergy, shortage, or formulary substitution
  • Familiarity with Drug I.D. and patient education resources
  • Clinical experience in ICU, oncology, paediatrics, geriatrics, or another speciality area
  • Prior data annotation, structured review, or AI evaluation experience
