Summary:
The Waiver Case Manager supports individuals with developmental disabilities by connecting them to Indiana's state-funded waiver services. This role involves conducting assessments, developing individualized service plans, coordinating multidisciplinary care, and ensuring compliance with Medicaid and state regulations. The Case Manager acts as a client advocate, promoting independence and well-being while maintaining accurate documentation and contributing to quality improvement initiatives.

Responsibilities:
  • Assess clients’ needs, strengths, and goals to determine eligibility.
  • Develop and regularly review Individualized Service Plans (ISPs).
  • Coordinate multidisciplinary care and maintain communication with stakeholders.
  • Ensure all documentation is accurate and compliant.
  • Monitor progress and adapt plans as needed.
  • Uphold Medicaid waiver program requirements.
  • Advocate for clients within healthcare and community systems.
  • Participate in training and quality improvement initiatives.
  • Collaborate with providers to deliver holistic support.


Requirements


  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field.
  • 3+ years of case management experience (preferably with developmental disabilities or Medicaid waivers).
  • Knowledge of Indiana waiver programs (HCBS), Medicaid regulations, and compliance standards.
  • Proficiency with EHR/case management software.
  • Excellent communication and organizational skills.
  • Valid driver’s license, reliable transportation.
  • Experience with person-centered and trauma-informed care.
  • Ability to manage multiple caseloads, prioritize, and meet deadlines.
  • Commitment to ethical practice and confidentiality.



Salary: $50,000 to $65,000
