Job description

Are you a GP looking for meaningful, flexible clinical work — fully remote, without the commute?

Proactive Care is seeking an experienced General Practitioner for a fully remote telehealth position, delivering structured chronic disease management for veterans and patients with complex, multi-system health needs.

This is a dedicated telehealth role with no in-clinic attendance required, and it's a long way from ten-minute, script-and-certificate telehealth. You'll run structured consultations for patients you follow over time, as part of an integrated Chronic Care Team, working alongside allied health professionals and specialists to keep complex patients on coordinated, evidence-based care plans. The work is purposeful, genuinely multidisciplinary, and built around thorough care rather than high-volume throughput.

We work across both Medicare and DVA billing streams, with a dedicated focus on veteran health and chronic disease management.

Remuneration

You start on a guaranteed hourly rate while you build your patient base, then move to a 70% share of billings once you're established. That gives you a secure floor from day one, with room to earn more as your list grows.

First 3 months (guaranteed):  $215.00 per hour

After 3 months:  70% of billings

  • Structured as an independent contractor arrangement
  • Bulk-billing model across Medicare and DVA billing streams
  • Sign-on bonus and anniversary incentive available (details on enquiry)

Location

This is a fully remote telehealth position. There is no physical clinic attendance required — all you need is a suitable home-office setup and a reliable internet connection.

The Role

As a General Practitioner within our Chronic Care Team model, you will:

  • Deliver telehealth consultations for chronic, complex and multi-system patients — assessment, review and ongoing management
  • Undertake chronic disease management (CDM) reviews and care planning, including GP Management Plans (GPMPs) and Team Care Arrangements (TCAs)
  • Integrate pathology, imaging and allied-health input into coordinated, evidence-based care plans
  • Manage prescribing, referrals and investigation requests as part of the coordinated care model
  • Collaborate with allied health professionals, specialists and care coordinators to optimise patient outcomes
  • Ensure clinical safety, prioritisation and escalation pathways are clearly identified and actioned

Working pattern: the role runs across the working week (Monday to Friday), with flexible scheduling available. All consultations are delivered via telehealth platforms.

Health & Focus

Proactive Care has a dedicated focus on health and chronic disease management for both Medicare and DVA-funded patients. You'll work with patients who often present with complex, multi-system conditions — chronic pain, mental-health comorbidities, musculoskeletal injuries and the long-term effects.

Requirements

Essential

  • Medical degree (MBBS or equivalent) and current registration with the Medical Board of Australia (AHPRA)
  • Fellowship with RACGP or ACRRM (VR GP), or eligibility for a 19AB exemption in an eligible DPA location
  • Experience in chronic disease management, preventative health and complex care coordination
  • Competence in prescribing, referrals and investigation management
  • Strong communication and collaboration skills within multidisciplinary teams
  • A suitable home-office environment and reliable internet connection for telehealth delivery

Desirable

  • Experience with telehealth consultations and digital-health platforms
  • Experience within team-based or multidisciplinary care models
  • Interest or experience in veteran health, DVA processes or the CVC Program
  • Experience with GP Management Plans (GPMPs), Team Care Arrangements (TCAs) and chronic disease care planning
