"The SNP Care Management team is responsible for the health care management and coordination of care for SNP members with complex and chronic care needs. The Supervisor, Care Coordination SNP, works with the SNP Manager to assure compliance with CM program requirements as outlined in the Model of Care. This includes the monitoring of activities to support the timeliness of Health Risk Assessment (Initial and Reassessment) completion, Individualized Care Plan (ICP) development and Interdisciplinary Care Team (ICT) delivery for SNP members. Additionally, the Supervisor, Care Coordination SNP, oversees the Care Coordinator SNP teamGeneral Duties/Responsibilities (May include but are not limited to):
SUPERVISORY RESPONSIBILITIES:
Oversees assigned staff. Responsibilities include recruiting, selecting, orienting, and training employees; assigning workload; planning, monitoring, and appraising job results; and coaching, counseling, and disciplining employees.
Minimum Requirements:

Minimum Experience:
3+ years Care Coordination experience; or any combination of education and experience, which would provide an equivalent background.
1 + years supervisory/lead experience in a health care setting or any combination of education and experience, which would provide an equivalent background.
Education/Licensure:
High school diploma or general education degree (GED); with one to three months related experience and/or training; or equivalent combination of education and experience.
No licensure required.
Other:
Ability to analyze data for trends.
Knowledge of Managed Care Plans and medical terminology
Computer Skills, 25 WPM (Microsoft Outlook, excel, word)
Good oral, written and telephone skills
Reasoning Skills: Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.
Computer Skills: Basic skills required.
Good organizational skills
Ability to reason and carry out instructions.
Good interpersonal skills.
Read, write and speak English fluently.
File systematically.

Pay Range: $58,531.00 - $87,797.00Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
