Your Opportunity 

Ready to step away from the frontline and into a role where your clinical expertise truly shapes patient outcomes? We’re offering a full‑time opportunity within our Aged Care team, supporting the growing Transition Care Program (TCP) and coordinating services that help older adults recover safely in their own homes. 

In this senior role, you’ll use your clinical judgement to guide care planning, manage any escalations and complex clients, and ensure every client receives a personalised, goal‑focused care experience. Your day will include: 

  • Completing initial phone assessments, care planning and goal setting
  • Coordinating in‑home services to keep clients safe and supported
  • Building strong relationships with primary and secondary care partners
  • Monitoring progress and updating care plans as needs change
  • Liaising with Local Health Networks and various health providers
  • Conducting home visits for assessments and admissions
  • Leading discharge planning and referrals to long‑term supports
  • Providing ongoing phone‑based education, advice and reassurance 

Hours and location 

  • Full-time permanent position
  • Monday to Friday (no shift work – family friendly hours)
  • Hybrid work (work from home or our Adelaide Hub)
  • Adelaide-based, with home visits mainly across metropolitan Adelaide  
  • Mileage reimbursement for travel

We’re looking for an experienced nurse who is: 

  • Clinically experienced (RN) in supporting consumers in the community  
  • Comfortable working both independently and as part of a multidisciplinary team, including remote collaboration
  • Experienced in community or aged care, with Transition Care Program knowledge highly regarded
  • An exceptional communicator who builds rapport easily with consumers, families and external agencies
  • Organised and proactive, with strong administrative skills
  • IT proficient and the ability to think creatively to solve problems
  • Self‑directed yet team‑minded, contributing to a supportive, positive and engaged team culture
