Your Role 

As the Program Manager, Pharmacy Operations with Evergreen Nephrology, you will lead the implementation, growth, and ongoing operational success of Evergreen's pharmacy program. This is a 6-month contract with the opportunity to convert to full-time employment. This program connects patients with a specialized pharmacy partner that provides pre-sorted, multi-dose medication packaging, home delivery, proactive refill management, and ongoing pharmacist support designed to improve medication adherence and simplify complex medication regimens for patients living with chronic kidney disease and other chronic conditions.  

In this role, you will provide leadership and oversight for a team of Pharmacy Care Coordinators responsible for patient outreach, enrollment, consent capture, documentation, and pharmacy workflow execution. You will oversee day-to-day program operations, establish standardized processes, monitor performance metrics, and ensure consistent execution across participating practices and markets.  

You will partner closely with provider practices, pharmacy partners, internal operational teams, and clinical stakeholders to support successful implementation, drive enrollment growth, remove operational barriers, and continuously improve the patient and provider experience. This role requires a balance of strategic thinking and hands-on operational leadership, with accountability for program performance, team development, implementation success, and operational excellence.  

While this position is fully remote, you must be able to work 8:30am – 5pm in the central or eastern time zone. This position will also require occasional travel to practice sites. 

Role Responsibilities 

Some responsibilities may vary based on specific patient programs, but this role's primary duties include the following: 

  • Oversee day-to-day operations of Evergreen's pharmacy program, ensuring consistent execution of enrollment, outreach, documentation, and pharmacy workflow processes.  
  • Lead, develop, and support a team of Pharmacy Care Coordinators through onboarding, training, coaching, performance management, and ongoing professional development.  
  • Establish and maintain operational workflows, standard operating procedures, training materials, scripts, and job aids that support consistent program execution and scalability. 
  • Monitor program performance against established enrollment, productivity, quality, and operational metrics; identify trends and implement improvement plans when needed.  
  • Develop and maintain program dashboards, reports, and performance summaries for operational and leadership stakeholders.  
  • Lead implementation activities for new provider practices, markets, and pharmacy program expansions, ensuring operational readiness and successful launch execution. 
  • Partner with provider practices, pharmacy partners, and internal Evergreen stakeholders to identify operational barriers, resolve issues, and improve program performance. 
  • Serve as the primary operational escalation point for complex patient, practice, provider, workflow, or pharmacy-related concerns. 
  • Conduct routine quality reviews, call audits, workflow assessments, and documentation reviews to ensure adherence to program standards, approved workflows, and compliance requirements.  
  • Analyze enrollment performance, workflow effectiveness, and operational outcomes to identify opportunities for process improvement and program optimization. 
  • Collaborate with pharmacy partners and internal operational teams to support effective communication, issue resolution, and alignment on program objectives. 
  • Support staffing planning, workforce allocation, and resource management to ensure appropriate program coverage and operational performance. 
  • Foster a culture of accountability, collaboration, continuous improvement, and exceptional service for patients, provider partners, and internal stakeholders.  
  • Uphold patient privacy and confidentiality standards in accordance with HIPAA and organizational policies. 
  • Other duties consistent with this role, as assigned. 

Required Qualifications 

  • Bachelor's degree in Healthcare Administration, Business Administration, Public Health, Nursing, or a related field. 
  • 4+ years of experience in healthcare operations, pharmacy operations, patient engagement, patient enrollment, care management support programs, population health, or a related healthcare environment. 
  • 2+ years of leadership experience managing teams, programs, projects, or operational initiatives. 
  • Experience leading high-volume patient outreach, enrollment, customer engagement, or operational workflow teams. 
  • Demonstrated experience developing, monitoring, and improving operational performance metrics and key performance indicators. 
  • Experience onboarding, coaching, developing, and evaluating team members. 
  • Experience working within electronic health record (EHR/EMR) systems and healthcare operational workflows.  
  • Strong analytical, organizational, and problem-solving skills, with the ability to translate data into actionable operational improvements. 
  • Exceptional communication, relationship-building, and stakeholder management skills. 
  • Execution-focused and hands-on, with the ability to effectively balance strategic priorities and day-to-day operational responsibilities.  
  • Working knowledge of HIPAA and healthcare privacy requirements. 
  • Intermediate skills with MS Office Suite of products including Outlook and Teams 
  • Able to work effectively in a primarily remote environment: 
    • Home internet must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps. Cable, Fiber, or DSL connections hardwired to the internet device are recommended 
    • Evergreen will provide remote employees with telephony applications and equipment to meet the business requirements for their role 
    • Employees must work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information 

 Preferred Qualifications 

  • Experience managing healthcare enrollment programs, patient engagement initiatives, population health programs, or pharmacy-related services. 
  • Experience leading implementations, practice launches, operational transformations, or new program deployments. 
  • Experience supporting medication adherence, pharmacy operations, prescription management workflows, or value-based care programs. 
  • Experience developing dashboards, performance reporting, and operational analytics.  
  • Experience working with nephrology, chronic kidney disease, ESRD, or other complex chronic disease populations. 
  • Experience managing external vendors, strategic partners, or healthcare service providers. 
  • Bilingual English/Spanish. 

Compensation 

The pay range for this role is $115,000-$120,000 annually. Exact pay is determined based on experience, education, demand for the role, and other role-specific factors.
