Job Summary

MGBfC strives to advance health equity, improve health outcomes, and promote the well-being of all patients by addressing health-related social needs and overcoming barriers to accessing care. The Community Health Worker Lead plays a critical role in achieving these goals by combining direct patient support with day-to-day leadership of the Community Health Worker team. Embedded within pediatric primary care, the Community Health Worker Lead collaborates closely with multidisciplinary care teams to support families with care coordination, assessment of unmet health-related social needs, and navigation to appropriate hospital and community resources. In addition to maintaining a patient-facing caseload, the Lead provides guidance, mentorship, training, and operational support to Community Health Workers, helping to ensure high-quality, equitable, and family-centered care. The Community Health Worker Lead serves as a resource to staff, promotes best practices in community-based care, assists with program coordination and workflow, and supports team operations, including work allocation and coverage in the absence of the Manager. While this is not a clinical role, it requires a strong understanding of or the ability to learn basic medical concepts, the ability to recognize when referral to a licensed clinician is appropriate, and a commitment to fostering the professional growth and success of the Community Health Worker team.


 

Qualifications

What you will do

  • Collaborate with healthcare providers, community organizations, and other stakeholders to develop and implement effective strategies to improve health outcomes
  • Ensure compliance with regulatory requirements and program standards
  • Collect and analyze data to measure program effectiveness and identify opportunities for improvement
  • Assist families to apply for social services, including Medicaid or Women, Infants, and Children (WIC).
  • Available for daily contact with CHWs to provide resource navigation peer support
  • Direct Patient/family to additional resources as needed
  • Updates and inputs progress notes, participant charts and EHR according to organizational standards.
  • Counsel clients in individual or group sessions to assist them in dealing with substance abuse, mental or physical illness, poverty, unemployment, or physical abuse.
  • Counsel or aid family members to assist them in understanding, dealing with, or supporting the client or patient.

Education Qualifications

  • Bachelor's Degree in Public Health required or
  • Bachelor's Degree in Social Work required or
  • Bachelor's Degree in Related Field of Study required

Experience in lieu of education may be considered

Experience Qualifications

  • Minimum 3-5 years experience in community outreach, health education, or related field required
  • Prior leadership experience strongly preferred

Skills and Abilities

  • Demonstrated ability to work effectively and provide advocacy for all populations and communities.
  • Strong communication and interpersonal skills, with the ability to interact effectively with various populations.
  • Ability to work independently and as part of a team.
  • Strong organizational and project management skills.
  • Basic computer skills, including Microsoft Office and database management.
  • Bilingual skills preferred.
