About the Role

We are seeking a Population Health Integrations Analyst to lead the development and execution of innovative population health strategies that improve access to care, reduce disparities, and enhance health outcomes across diverse populations.
In this role, you will serve as a strategic leader driving the integration of population health principles into care delivery, outreach, education, and care coordination programs. You will collaborate across clinical, quality, operational, and community-based teams to ensure compliance with regulatory requirements while delivering programs that support preventive care, member engagement, and continuity of services.
Please note: This role is focused on population health strategy, care integration, program implementation, community outreach, and cross-functional leadership. While data and performance metrics are used to inform decision-making, this is not a data analyst or reporting-focused position and does not primarily involve building reports, dashboards, or performing advanced statistical analysis..
What You'll Do
- Develop and lead enterprise-wide strategies for health promotion, preventive outreach, and population health improvement initiatives.
- Oversee care integration programs supporting high-risk, vulnerable, and special-needs populations.
- Drive community outreach and health education efforts, ensuring culturally and linguistically appropriate engagement.
- Partner with Quality, Clinical, Operations, Provider Relations, and Member Services teams to design and implement interventions that improve utilization, close care gaps, and reduce health disparities.
- Utilize data analytics, predictive indicators, and utilization trends to identify target populations and develop tailored intervention strategies.
- Build and maintain relationships with community organizations, schools, public health agencies, providers, and other key stakeholders to expand access to preventive services and health education.
- Lead the development of educational materials, outreach campaigns, and community engagement initiatives that improve health literacy and promote early intervention.
- Ensure compliance with NCQA Population Health Management (PHM) standards, CMS requirements, state regulations, and contractual obligations.
- Monitor program performance, evaluate outcomes, and present findings, trends, and recommendations to executive leadership and quality committees.
- Lead continuous improvement initiatives, including process optimization, staff development, and implementation of innovative technologies that enhance member engagement and care coordination.
What You'll Bring
Education
- Bachelor's degree in Public Health, Healthcare Administration, Nursing, or a related field preferred.
- Equivalent combination of education and relevant experience may be considered.
Experience
- Minimum of 2 years of experience in population health management, care integration, health promotion programs, or a related healthcare environment.
- Experience working with NCQA PHM standards, CMS regulations, and state compliance requirements.
Knowledge, Skills & Abilities
- Strong ability to lead cross-functional projects, manage multiple priorities, and maintain audit-ready documentation and processes.
- Deep knowledge of NCQA, URAC, CMS, and state quality and regulatory requirements, with the ability to translate standards into operational practice.
- Experience designing, implementing, and evaluating Quality Management and Performance Improvement programs.
- Strong analytical skills with the ability to interpret performance metrics, identify trends, assess risk, determine root causes, and implement sustainable improvements.
- Excellent communication and relationship-building skills with the ability to influence stakeholders across clinical, operational, and compliance teams.
- Demonstrated ability to supervise, mentor, and develop high-performing teams while fostering accountability and continuous improvement.
