What will you do as a PMC Medical Assistant - Emergency Care Coordinator:

Emergency Care Coordinator (ECC) is a critical role in ensuring that value-based care patients who have been discharged from the emergency room receive the necessary follow-up support for their recovery. The Emergency Care Coordinator will proactively contact discharged patients to ensure they understand their discharge instructions, provide ongoing symptom monitoring, and assess their need for further medical support, including primary care appointments and medications.  Check out what its like to be a  Medical Assistant at PMC.

 

A day in the life of a PMC Medical Assistant: Emergency Care Coordinator may include:

· Post-Discharge Outreach: Reach out to patients who have been discharged from the emergency room to confirm they understand their discharge instructions and to answer any questions they may have.

· Quality FMC Measure: Ensure that a two-way communication note is completed on every patient, in order to also capture the patients that fall into the FMC quality gap.

· Symptom Monitoring: Check in with patients regarding any continued symptoms or new concerns following their emergency room visit, ensuring they are properly managing their recovery.

· Care Coordination: Determine if patients need to schedule follow-up primary care appointments or additional support for their recovery and assist them with scheduling as needed.

· Medication Review: Confirm that patients have received their prescribed medications, understand their instructions for use, and address any concerns, escalating to the appropriate team member as needed.

· Patient Education: Provide education on when and how to contact their primary care provider, how to access walk-in clinic, and emphasize the importance of avoiding unnecessary emergency room visits.

· Resource Navigation: Assist patients in identifying available resources, including community support or services that can aid in their recovery process.

· Documentation: Maintain accurate and detailed records of patient interactions, including follow-up calls, symptoms reported, medications, and scheduled appointments.

· Collaboration: Work closely with the healthcare team, including physicians, nurse practitioners, and other care coordinators, to ensure seamless patient care and timely follow-ups.


 Required Qualifications

High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

Certification through any of the following regulatory bodies is required:  CMA (Certified Medical Assistant) – American Association of Medical Assistants, RMA (Registered Medical Assistant) – American Medical Technologists, CCMA (Certified Clinical Medical Assistant) – National Healthcare Association, or NCMA (National Certified Medical Assistant).

Current CPR/BLS Certification or be able to obtain certification within 90 days of hire.


Preferred Qualifications

Prior experience in a healthcare setting, particularly in care coordination, patient support, or nursing, is highly desirable.

Familiarity with value-based care models and patient-centered care principles is a plus.

 

Shift: Day Shift (Monday through Friday) no weekends or holidays

Pay Type: Hourly (Non-Exempt)
