Coordinated Specialty Care Implementation Manager

The Role: The Coordinated Specialty Care (CSC) Implementation Manager is responsible for coordinating and managing the development of an implementation-ready Coordinated Specialty Care (CSC) program model for Vermont. Working under the direction of the Director of Strategic Initiatives, this position oversees project planning, stakeholder engagement, coordination of consultants and subject matter experts, development of implementation materials, and completion of contract deliverables required by the Vermont Department of Mental Health. 


The Implementation Manager ensures activities remain on schedule, aligned with evidence-based CSC practices endorsed by SAMHSA, and responsive to the realities of rural service delivery.


The anticipated project period is August 1, 2026 - July 31, 2027.

Schedule: Full-Time, time-limited (through July 2027)

Responsibilities:

Program Design and Development Coordination:

  • Coordinate the development of an implementation-ready program model for a Coordinated Specialty Care (CSC) program in Vermont in consultation with content experts, service providers, individuals who have experienced psychosis and their families, and other stakeholders as a first phase toward statewide implementation.

  • Coordinate the development of operational supports for program delivery, including program policies and procedures, incoming and outgoing referral processes, admission criteria, discharge protocols, job descriptions, and advisory board structures


Stakeholder Engagement and Partnership Development:

  • Coordinate stakeholder engagement activities and support the implementation and development of a stakeholder engagement and advisory board plan 

  • Recruit and convene stakeholders, including national content experts, individuals who have experienced psychosis and their families, and others as necessary for advisement and input into program implementation

  • Support engagement of community leaders and organizations to promote and support the implementation of a future CSC team

  • Engage community partners and establish necessary partnerships for program implementation including the exploration of MOUs or subrecipient agreements 

Training and Community Education:

  • Work with Pathways Vermont Training Institute, content experts, and existing national models to develop a comprehensive Coordinated Speciality Care Training Curriculum that incorporates best practice requirements 

  • Prepare and deliver outreach and educational materials and engage referral partners

  • Participate in messaging that aligns with values and best practices and provides opportunities for community education and participation

Project Management: 

  • Coordinate the development and successful execution of all contract deliverables including:

    • Prepare an implementation plan report describing the proposed service delivery model, staffing structure, governance, recommended pilot site, referral pathways, and incorporation of stakeholder feedback into the final model

    • Prepare an implementation plan blueprint to the state that describes the operational approach for launching a single-site CSC pilot team

    • Prepare a Coordinated Specialty Care training plan and curriculum

    • Prepare a policies and procedures manual for Coordinated Specialty Care teams

    • In coordination with Pathways leadership, ensure the preparation and completion of a financing and billing recommendations report to the state 

    • In coordination with Pathways leadership, deliver above materials to the state in full and on time 

  • Develop and maintain project timelines and work plans 

  • Track milestones, deliverables and contract requirements 

  • Coordinate meetings, agendas, and follow ups

  • Monitor project risks and escalate issues to agency leadership

  • Coordinate project consultants, stakeholders, and subject matter experts

  • Coordinate internal communication related to CSC project 


  • Other Duties as assigned

  • Attendance at the annual All Staff and Programmatic Retreats is an expectation of employment with Pathways Vermont, unless excused by the employee’s manager/HR. Retreats are held on workdays, during normal working hours, and are fully paid.


Qualifications:

  • At least 3 years project management and/or program implementation experience

  • Background in administration, database management, computer programs (including Microsoft Office Suite, Google Apps, Electronic Medical Records, Internet browsers, etc.)

  • Must be able to work in a fast paced environment as well as possess strong problem solving skills, work ethic and computer proficiency

  • Must be able to work flexible hours

  • Driver’s license, registration, insurance and vehicle required

  • Out of state travel for the purposes of evaluating CSC programs may be occasionally required

  • Excellent verbal and written communication 

  • Must be comfortable with rapid growth and thinking out of the box

  • Excellent organizational skills including project management experience 

  • Ability to speak to a large audience

  • Personal experience of mental health challenges including extreme state experiences will be considered an asset


Work Conditions/Physical Demands:

  • Almost constant operation of a computer or other office machinery

  • Regular, repetitive movement of hands and fingers including substantial movement of wrists, hands and fingers for typing and/or writing, grasping paperwork, using technology equipment and other tasks

  • Frequent utilization of close visual acuity reading from a computer screen, most be able to read information captured in type and free hand form

  • Frequent talking and participation in exchanging ideas through spoken word as well as ability to hear and receive detailed information through oral communication

  • Mostly sedentary work in a seated position

  • Occasional travel by vehicle, including the possibility of occasional long distance driving

  • Occasional standing, walking, stooping, kneeling or crouching reaching with hands and arms and light lifting

  • Occasional exposure to outside environmental conditions for community-based meetings

  • Comfort initiating new relationships

  • Regular attendance is a requirement of the position 


Compensation:

  • $60,000 - $70,000 annual salary 

  • Anticipated position term is 8/1/26 - 7/31/27
