Job description

Our Client, a Healthcare company, is looking for a Pharmacist for their Remote location.
 
Responsibilities:
  • Job Description attached, main duty is to review RX's for Therapeutic Interchanges
  • Accept and dispense prescription medications.
  • Monitor patient medication compliance and over-usage patterns.
  • Ensuring that the supply of medicines is within the law.
  • Ensuring that the medicines prescribed to patients are suitable.
  • Accurately maintain confidential records of all patients ensure proper management of patient records and pharmacy files.
  • Advising patients about medicines, including how to take them, what reactions may occur and answering patients' questions.
  • Provide consultation on prescription and non-prescription medications for storage, dosage, side effects, and drug interactions.
  • Provide services to patients, such as smoking cessation, blood pressure measurement and cholesterol management.
  • Educate patients on proper handling of medical devices such as blood pressure monitor and glucose meters.
  • Compound medications for external or internal use.
  • Prepare sterile solutions following proper procedures.
  • Supervise pharmacy staff members including pharmacy technicians, pharmacy clerks, etc.
  • Maintain appropriate inventory on pharmaceutical and medical supplies.
  • Identify patient's drug-related problems and effectively communicate with physicians and other healthcare practitioners.
  • Work in conjunction with physicians, nurses, other pharmacists, pharmacy interns, etc.
  • Doctor of Pharmacy degree required.
  • Licensed by the state pharmacy board.
  • Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
  • Makes recommendations for therapy changes to the physician and facility staff as appropriate
  • Maintains patient/resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies)
  • Ensures that appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated, and discussed with appropriate members of the healthcare team
  • Collaborates with the healthcare team to provide clinical and technical support as needed
  • Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record
  • Obtains, as necessary, and maintains all required documentation in accordance with federal and state regulatory agencies
  • and policy
  • Informs the Pharmacy in Charge (P.I.C) of medication related errors
  • Assists facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs
  • Performs other tasks as assigned
  • Works with Management on special projects as assigned.
  • Conducts job responsibilities in accordance with the standards set out in the Company’s Code of Business Conduct and Ethics, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards.
