About the Role

We are seeking an experienced Chronic Care Management (CCM) Specialist - Patient Ally Proficiency to join a growing healthcare team. This role is ideal for someone with hands-on U.S. CCM experience who is passionate about supporting patients through ongoing care coordination and proactive follow-up.

As a CCM Specialist, you will play a key role in improving patient outcomes by conducting routine follow-up calls, documenting patient interactions, coordinating care activities, and maintaining accurate patient records. The ideal candidate is organized, self-motivated, bilingual, and comfortable working independently in a fully remote environment.

Key Responsibilities

  • Conduct Chronic Care Management (CCM) follow-up calls with eligible patients.
  • Build positive relationships with patients and encourage ongoing engagement in their care plans.
  • Accurately document all patient interactions and care activities within established timelines.
  • Maintain and update CCM tracking logs using Google Drive and Google Sheets.
  • Utilize Office Ally as the primary Electronic Health Record (EHR) system.
  • Coordinate patient care activities and ensure all required documentation is complete and compliant.
  • Monitor patient progress and escalate concerns to the appropriate clinical team when necessary.
  • Collaborate with providers and other healthcare professionals to support continuity of care.
  • Maintain strict HIPAA compliance and protect patient confidentiality at all times.
  • Perform additional administrative and care coordination duties as assigned.

Required Qualifications

  • 1–2+ years of recent hands-on experience in U.S. Chronic Care Management (CCM).
  • Experience conducting patient follow-up calls and coordinating ongoing patient care.
  • Proficiency with Google Drive and Google Sheets for patient tracking and workflow management.
  • Experience using Office Ally EHR.
  • Strong understanding of U.S. healthcare workflows, documentation standards, and patient communication.
  • Excellent verbal and written English communication skills.
  • Strong organizational skills with exceptional attention to detail.
  • Ability to work independently, manage priorities effectively, and thrive in a remote environment.
  • Bilingual proficiency is required.

Preferred Qualifications

  • Experience using Patient Ally.
  • Experience supporting Remote Patient Monitoring (RPM) or other care coordination programs.
  • Medical Assistant, Nursing, or other clinical healthcare background.
  • Familiarity with insurance documentation and patient scheduling processes.

Technical Requirements

  • Reliable high-speed internet connection.
  • Quiet, professional home office environment.
  • Computer capable of running multiple browser-based healthcare applications simultaneously.

Schedule & Availability

  • Full-time remote position.
  • Availability to work during U.S. business hours.

What We're Looking For
