Job description

The Clinical Care Coordinator is a licensed behavioral health clinician who serves as the first clinical point of contact for individuals seeking care at Salma Health. This role combines crisis assessment, clinical intake, treatment navigation, admissions support, and ongoing patient engagement to ensure patients receive timely, compassionate, and appropriate care throughout their journey.

Working closely with our Care Navigation, Provider, and Clinical Operations teams, the Clinical Care Coordinator helps guide patients from their initial outreach through admission, treatment initiation, and post-treatment follow-up. This role plays a critical part in expanding access to high-quality brain and mental healthcare while ensuring every patient receives personalized, evidence-based support.

Key Responsibilities

  • Serve as the primary clinical contact for prospective patients, providing compassionate guidance and support during their initial interaction with Salma Health.

  • Conduct comprehensive clinical assessments, including risk evaluations, safety planning, and triage for individuals presenting with behavioral health concerns.

  • Manage crisis calls and high-risk clinical situations, including suicidality, mandated reporting, and escalation to appropriate emergency resources when indicated.

  • Educate patients and families on Salma Health's services and treatment options, helping match patients to the most appropriate level of care.

  • Complete clinical intake and admissions activities, including gathering psychiatric history, reviewing documentation, and coordinating next steps for treatment initiation.

  • Collaborate closely with psychiatrists, therapists, care coordinators, and operational teams to ensure seamless transitions throughout the patient journey.

  • Conduct post-treatment outreach and follow-up assessments to evaluate patient outcomes, identify ongoing care needs, and support continuity of care.

  • Accurately document all patient interactions, assessments, and clinical recommendations in the electronic health record.

  • Support quality improvement initiatives by identifying trends, providing clinical feedback, and contributing to operational improvements that enhance patient experience and access to care.

  • Maintain compliance with all applicable clinical, regulatory, privacy, and documentation standards.

Required Qualifications

  • Master's degree in Clinical Social Work, Counseling, Marriage & Family Therapy, Nursing, or a related behavioral health field.

  • Active, unrestricted independent clinical licensure required (LCSW, LMFT, LPCC, LPC, or equivalent). Registered Nurses with applicable psychiatric experience will also be considered.

  • Minimum of 2 years of post-licensure clinical experience.

  • Minimum of 2 years of experience conducting psychiatric intake, behavioral health assessments, crisis intervention, or related clinical work.

  • Demonstrated experience managing high-risk behavioral health situations, including suicide risk assessment, crisis intervention, and mandated reporting.

  • Excellent clinical judgment, communication, and active listening skills, with the ability to remain calm and empathetic in fast-paced, high-acuity situations.

  • Strong organizational skills with exceptional attention to detail and documentation accuracy.

  • Experience working with electronic health record (EHR) systems and clinical documentation platforms.

  • Must be physically located within the United States. Work may not be performed outside the United States.

Preferred Qualifications

  • Experience in behavioral health call centers, Employee Assistance Programs (EAP), psychiatric admissions, or centralized intake environments.

  • Experience in outpatient psychiatry, crisis services, interventional psychiatry, or integrated behavioral healthcare.

  • Bilingual in Spanish strongly preferred.

Schedule Requirements

Candidates must be available to work one of the following full-time schedules (with a lunch break):

  • Tuesday – Saturday, 9:30 AM – 6:00 PM PT

  • Sunday – Thursday, 9:30 AM – 6:00 PM PT

Compensation & Benefits

The compensation for this position includes:

  • Hourly Rate: $33.00–$38.00 per hour, commensurate with location, experience and qualifications.

  • Benefits: Medical, dental, and vision insurance; Paid Time Off (PTO) and paid holidays; additional employee benefits.
