Job description

Haven Headache & Migraine Center | Remote (California Licensed) | Contract / 1099 Partnership | Immediate Start

Haven is building the future of migraine care for the 40 million Americans who suffer from this debilitating neurological condition. Backed by Precursor Ventures and 1843 Capital, we're transforming how migraine is treated through an innovative combination of telemedicine, AI-enabled monitoring, and personalized care plans. Our early results are exceptional—our initial patient cohort is seeing a 70% improvement rate with days impacted by headache reduced by half.

Sleep and migraine are deeply intertwined, and sleep medicine is a core pillar of how we treat the whole patient. We are expanding our clinical team to bring comprehensive, telehealth-based sleep care to patients across California.

The Opportunity

As an early contract Advanced Practice Provider (NP or PA) partnering with our Sleep Medicine team, you will have a direct, meaningful impact on patients’ lives. You'll manage a clinical panel of patients with sleep disorders that commonly travel with migraine — insomnia, obstructive sleep apnea, and circadian rhythm disorders — in close collaboration with our integrated care team led by our Chief Medical Officer and Medical Director of Sleep.

This is a unique chance to bring your specialized expertise to a clinic designed around true patient outcomes, not fee-for-service churn. If you’re energized by patient connection, evidence-based care, and scaling a sleep program from the ground up inside a fast-growing, venture-backed startup, this partnership is a perfect fit.

Scope of Services:

Your primary focus will be providing exceptional clinical evaluation for sleep disorders and collaborating closely with our headache specialists to deliver integrated, whole-patient outcomes.

Key clinical functions include:

  • Conducting independent telehealth visits for the evaluation and ongoing management of sleep disorders.

  • Ordering and interpreting home sleep apnea tests.

  • Managing PAP therapy initiation, titration, and long-term adherence.

  • Prescribing and managing digital therapeutics for insomnia (including CBT-I-based programs) and pharmacologic treatments when clinically indicated.

  • Collaborating closely with our headache specialists to seamlessly coordinate care for patients with comorbid migraine.

  • Documenting clinical findings thoughtfully and advising on clinical protocols as we scale the sleep program.

What We're Looking For

  • An active, unrestricted California license as a Nurse Practitioner (FNP, AGNP, or PMHNP) or Physician Assistant, with active DEA registration.

  • 2+ years of experience in sleep medicine (or strong sleep exposure in pulmonology, neurology, or primary care).

  • Comfort navigating home sleep testing workflows, PAP management, and insomnia care.

  • Experience or high comfort level with Telehealth, virtual care, and building real rapport with patients through a screen.

  • Comfort navigating early-stage startup ambiguity and evolving clinical workflows.

  • A genuine passion for helping patients feel seen, supported, and understood.

Nice to haves: Active clinical licenses in Texas or Washington; experience with digital therapeutics (e.g., prescription CBT-I); prior exposure to headache medicine.

Location Information

This is a remote partnership. While you can be physically located anywhere within the United States, you must hold active, unrestricted clinical credentials in the state of California to treat our patient population.

Project Volume & Capacity

This is a 1099 contract partnership. To support our immediate program launch and ongoing operations, we anticipate a flexible project volume requiring an estimated 8 to 30 hours per week, depending on patient scheduling and your availability. As our patient population expands, there is significant opportunity to scale this volume and take on additional clinical capacity in the future.

Our Culture & Fees

At Haven, we offer a unique opportunity to work alongside experienced founders who have built and scaled successful healthcare companies. As an independent contractor, you will be compensated via a competitive hourly contract rate or per-case flat fee (TBD based on experience).

Diversity & Inclusion

Diversity is built into our DNA with a founding team representing POC, female, disabled, and LGBTQ+ founders. We strongly encourage clinicians from all backgrounds to apply — many successful partners may not meet every single preference listed. We'd love to hear from you even if you don't check every box.

Haven Headache and Migraine Center is proud to partner with a diverse network of independent professionals. Contract awards are based solely on clinical merit, performance, and business needs.
