Job Summary
We are seeking a Care Management Platform Consultant to support a health plan client through the final phases of a care management platform implementation. This role will focus on workflow design, system configuration, testing support, and go-live readiness.
The ideal candidate has experience working with care management platforms such as Helios, GuidingCare, Jiva, CareProminence, CareAdvance, or similar systems and can translate business and clinical workflows into system functionality.
Responsibilities
Support workflow design, configuration, and validation for a care management platform implementation
Translate operational and clinical requirements into system behavior and configuration
Configure and validate rules, triggers, member identification logic, cohorts, workflows, and related functionality
Partner with client teams during testing to troubleshoot issues and confirm expected outcomes
Support UAT, defect triage, and go-live readiness activities
Facilitate discussions with clinical, operational, and technical stakeholders
Identify gaps between requirements, workflow design, and system configuration
Provide practical recommendations to improve workflow efficiency and platform usability
Support implementation of new platform functionality as it is released
Required Qualifications
5+ years of experience supporting care management, utilization management, population health, or health plan operations
Hands-on experience with at least one care management platform, such as:
Helios / VirtualHealth
GuidingCare
Jiva / ZeOmega
CareProminence
CareAdvance / MHK
Similar care management or population health platforms
Experience translating workflows into system configuration
Strong understanding of rules-based workflows, triggers, member identification, cohorting, or care management automation
Experience supporting testing, UAT, issue resolution, or implementation readiness
Strong client-facing communication skills
Preferred Qualifications
Prior Helios / VirtualHealth experience
Clinical background such as RN, LCSW, or similar
Experience supporting Medicare, Medicaid, or commercial health plan populations
Prior experience with late-stage implementations, go-live support, or system stabilization
Ability to work across business, clinical, and technical teams
