Description

The Healthcare Transformation Advisor provides strategic subject matter expertise supporting healthcare transformation, public health modernization, rural health initiatives, and government-funded healthcare programs. This role advises program leadership, operational teams, and stakeholders on healthcare delivery transformation, care coordination strategies, population health initiatives, operational modernization, and program implementation approaches aligned with federal and state priorities.


Working closely with program leadership, evaluation teams, operational staff, and client stakeholders, the Healthcare Transformation Advisor supports strategic planning, operational improvement efforts, and healthcare transformation activities designed to improve healthcare access, quality, service integration, and population health outcomes.


This role requires strong healthcare domain expertise, strategic thinking, and the ability to translate healthcare policy, operational considerations, and transformation objectives into practical recommendations and actionable guidance. The ideal candidate brings experience supporting healthcare transformation initiatives, Medicaid programs, public health operations, healthcare delivery systems, or government-funded healthcare programs.


This role may support Rural Health Transformation Programs (RHTP) and other state-led healthcare transformation initiatives focused on improving healthcare access, quality, delivery system performance, care coordination, and population health outcomes.


Key Responsibilities

Healthcare Transformation & Strategic Advisory Support

  • Provide subject matter expertise related to healthcare transformation, rural health initiatives, healthcare delivery systems, and public health operations. 
  • Advise program leadership and stakeholders on healthcare program implementation strategies, operational models, and care coordination approaches. 
  • Support strategic planning and operational decision-making associated with healthcare transformation and public health modernization initiatives. 
  • Provide guidance related to healthcare access, population health, health equity, service integration, and community health improvement strategies. 
  • Support evaluation of healthcare delivery models, operational effectiveness, and transformation outcomes. 

Program & Stakeholder Support

  • Contribute expertise related to Medicaid programs, Centers for Medicare and Medicaid Services (CMS) initiatives, public health systems, and government-funded healthcare programs. 
  • Participate in governance activities, stakeholder meetings, planning sessions, and strategic discussions. 
  • Support development of reports, presentations, recommendations, and stakeholder-facing deliverables. 
  • Collaborate with operational, policy, technical, analytical, and clinical stakeholders to support program objectives. 
  • Support initiatives focused on rural healthcare access, care coordination, healthcare modernization, and community health outcomes. 

Operational & Strategic Collaboration

  • Collaborate across cross-functional teams to support alignment between operational activities, program objectives, and healthcare transformation priorities. 
  • Provide guidance on healthcare operational best practices, implementation considerations, and program improvement opportunities. 
  • Support identification of operational risks, barriers, and opportunities associated with healthcare transformation initiatives. 
  • Contribute to continuous improvement activities and strategic program discussions. 

Team Collaboration

  • Demonstrate professionalism, accountability, responsiveness, and strong strategic thinking capabilities. 
  • Collaborate effectively across diverse stakeholder groups in a fast-paced and highly collaborative environment.

Requirements

Education & Experience

  • Bachelor’s degree required in Public Health, Healthcare Administration, Nursing, Health Policy, Healthcare Management, Social Work, or a related field. 
  • Master’s degree preferred in Public Health, Healthcare Administration, Nursing, Health Policy, Healthcare Management, or a related discipline. 
  • Minimum of 7 years of experience supporting healthcare transformation initiatives, public health programs, healthcare operations improvement efforts, Medicaid programs, or government-funded healthcare initiatives. 
  • Experience supporting healthcare modernization, care coordination, population health, public health, or rural health initiatives preferred. 
  • Experience supporting state or federal healthcare programs, public health agencies, healthcare systems, or government-funded healthcare environments preferred. 
  • Experience supporting healthcare operations, policy implementation, or transformation initiatives within public sector environments preferred. 

Required Knowledge, Skills & Abilities

  • Strong knowledge of healthcare delivery systems, healthcare transformation initiatives, and public health operations. 
  • Understanding of Medicaid programs, Centers for Medicare and Medicaid Services (CMS) initiatives, healthcare modernization efforts, and government healthcare environments. 
  • Ability to translate healthcare policy and operational concepts into practical recommendations and actionable guidance. 
  • Strong verbal and written communication skills with the ability to engage effectively with leadership, stakeholders, and cross-functional teams. 
  • Experience supporting healthcare operations improvement, care coordination, population health, or healthcare access initiatives. 
  • Strong collaboration, facilitation, stakeholder engagement, and relationship management skills. 
  • Ability to manage multiple priorities and support fast-paced healthcare transformation initiatives. 
  • Strong analytical, organizational, and problem-solving capabilities. 
  • Proficiency with Microsoft Office Suite including Word, Excel, PowerPoint, Teams, and SharePoint. 

Work Schedule

Consistent, full-time work expected Monday – Friday, eight hours per day in a focused and fast-paced environment.
