General Statement:
Under the direction of the Supervisor, and as part of a team, provides case management and/or supports coordination in the home, office, and community for children/adolescents with serious emotional
disturbances (SED), developmental disabilities, and/or co-occurring disorders; develops the person/family centered plan with the child, family and natural supports; assesses, links, monitors, coordinates services; assists families in working with the school systems, the court, child welfare and other community resources; works with the family, other professionals and natural support networks to provide support and structure for the child in the home and community; performs related work as required.

Bachelor's Degree in Social Work
| Michigan Bachelor Level Social Worker (LBSW) credential. Will consider a candidate with a Michigan Limited License Bachelor Level Social Worker (LLBSW) credential. | Three (3) years experience in the evaluation and treatment of children or adolescents. | The employee with a LLBSW must take and pass the ASWB Examination within six (6) years of hire to retain employment. Upon passing the ASWB exam, the employee must immediately apply for the LBSW credential. |
Bachelor’s degree in a human service field (defined as Education, Counseling/Guidance or Psychology). |
| Three (3) years experience in the evaluation and treatment of children or adolescents. |

Employee must maintain valid professional licensure to retain employment.
Additional Requirements:
Must retain DCH qualifications as a Child Mental Health Professional.
Must have a valid Michigan driver’s license, current automobile insurance, and the use of a vehicle during work hours. Ability to physically perform the tasks.
