RN Care Manager PART TIME – Babyscripts About Babyscripts

Babyscripts is a leading innovator in digital health, dedicated to transforming maternal care through remote patient monitoring (RPM) and patient engagement. Our digital tools help healthcare providers monitor, educate, and support their patients in real-time, enhancing overall care coordination and improving maternal and fetal health outcomes.

About the Role

We are seeking a part-time RN Care Manager to support maternal health patients through care coordination, remote patient monitoring, and clinical assessment. This role is ideal for licensed Registered Nurses (RNs) with experience in case management, maternal health, or telehealth who are looking for a flexible position in a digital care environment.

As a part-time team member, you will monitor patient data, conduct clinical assessments, educate patients, and escalate high-risk cases to providers using established protocols. You’ll play an important role in improving maternal health outcomes—while working remotely with a flexible schedule aligned to business needs.

This is a remote, part-time (approximately 24 hours/week) position, with some flexibility around days and hours.

Key Responsibilities

  • Monitor patient data trends (e.g., blood pressure, weight, symptoms) and identify high-risk cases requiring intervention.
  • Perform clinical assessments via phone, telehealth, or messaging to provide education, intervention, and care plan adjustments.
  • Provide direct patient education on managing hypertension, gestational diabetes, preeclampsia risks, postpartum recovery, and other maternal health concerns.
  • Escalate high-risk cases to the appropriate provider per established protocols.
  • Support care coordination by assisting patients with referrals, scheduling follow-ups, and connecting them to additional resources.
  • Ensure compliance with RPM protocols by coaching patients on adherence and addressing barriers to engagement.
  • Document all clinical interactions in Babyscripts' care management platform, ensuring accuracy and compliance with healthcare regulations.

Preferred Qualifications & Experience

  • Active, unrestricted RN license in the U.S. (multi-state compact license preferred).
  • Minimum of 3 years of clinical experience in maternal health, OB/GYN, case management, remote patient monitoring, or other relevant fields.
  • Experience in telehealth, digital health, or RPM platforms is highly preferred.
  • Strong critical thinking and decision-making skills to manage maternal health risks effectively.
  • Excellent patient communication skills with the ability to educate, engage, and empower patients in their care.
  • Proficiency with EHRs, care management platforms, and digital health tools.
  • Bilingual (Spanish preferred).

Compensation & Benefits

  • Salary: Based on experience and licensure; part-time compensation is pro-rated based on a 24-hour workweek.
  • Quarterly Bonuses: Eligible based on patient engagement, quality benchmarks, and documentation accuracy (pro-rated based on hours worked).
  • Flexible Scheduling: Self-managed hours in coordination with team needs and coverage goals.
  • Remote Work:
    • Work-from-home stipend available.

Note: Part-time employees are not eligible for full-time medical, dental, vision, or 401(k) benefits, but may be eligible for select company perks depending on hours worked and tenure.

Staffing & Coverage Expectations

  • Maintain a consistent 24-hour/week schedule, with flexibility around days and hours based on team coverage needs.
  • Coordinate any changes in availability or time off in advance to ensure patient continuity.
  • In the case of urgent PTO, coordinate handoffs with the team to ensure no disruption in patient care.
