We are currently recruiting for a Nurse Practitioner.

Job Description

The Nurse Practitioner reports to the Director for administrative supervision and the Practice Site Physician or designee for clinical supervision. Is responsible for the coordination and delivery of primary care, as specified by the appropriate certifying agency, to patients of specific area employers. Provides clinical and administrative assistance to programs, etc. Works collaboratively to meet clinic objectives for patients and clients.

Qualifications

Working within the approved scope of practice as designated for this type of professional.

Obtaining complete medical history and physical data on patients.

Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures as needed.

Performing a complete physical exam and recording findings. Managing acute and chronic medical needs of patients.

Ordering appropriate laboratory and diagnostic procedures.

Synthesizing data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.

Administering, dispensing, and prescribing medications and injections.

Interviewing and advising patients regarding health and illness prevention. Recommending community resources to meet patient and family needs.

Instructing patient and family regarding medications and treatment instructions.

Maintaining and reviewing patient records, charts, and other pertinent information. Posting tests and examination results.

Triaging patient telephone calls and providing consultation.

Providing monitoring and continuity of care as appropriate between visits.

Collaborating with a Medical Physician in accordance with professional guidelines.

Clinical and Administrative Assistance

Participating in health education.

Maintaining related records and statistical data and preparing statistical reports.

Participating in interdisciplinary care conferences and assisting with referrals and dissemination of information to appropriate care partners.

Assisting in the development and coordination of ongoing education and followup programs designed to measure client outcomes.

Competence and Clinc Effectiveness

Developing and maintaining affiliation with professional and community organizations.

Assisting in development and compliance of clinical protocols, policies and procedures.

Participating in quality review and improvement programs; monitoring quality and outcomes of care rendered.

Performing other jobrelated assignments and special projects as directed.

Organizational Requirements

Attends and participates in department meetings and is accountable for all information shared.

Completes mandatory education, annual competencies and department specific education within established timeframes.

Completes annual employee health requirements within established timeframes.

Maintains licensecertification, registration in good standing throughout fiscal year.

Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by positiondepartment.

Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self.

Adheres to regulatory agency requirements, survey process and compliance.

Complies with established organization and department policies.

Available to work overtime in addition to working additional or other shifts and schedules when required.

Education and Experience

The knowledge, skills and abilities as indicated below are normally acquired through the successful completion of a Bachelors degree in Nursing. A valid and current RN license in the state of Indiana is required. The successful completion of a nurse practitioner program with applicable national certification is required. Two years of clinical nursing experience in practice area and successful completion of a Nurse Practitioner six month internship for new graduates or comparable experience is required. Masters of Science Degree in Nursing is preferred. Attainment of prescriptive authority or successful completion of applicable course work and pass state board exam to obtain prescriptive authority within six months of hire date. Bilingual speaking andor writing skills are a plus.

Knowledge & Skills

Requires a thorough knowledge of practice standards and policies and procedures of MMG and the appropriate professional organization.

Requires a high level of analytical skills necessary to assess the patient and participate in followup care planning.

Demonstrates interpersonal skills necessary to effectively interact with patient(s), family of patient(s), physicians, other professionals and paraprofessionals.

Demonstrates high level of communication skills, both oral and written, necessary for effective communication with patientfamily, physicians, and referral sources.

Working Conditions

Works in a clinic environment requiring physical exertion, frequent changes in job demands, certain undesirable patient care activities, and exposure to biohazards.

Consequence of errors in performance may result in patient injury or death.

Physical Demands

Requires the physical ability and stamina (i.e. to walkstand for prolonged periods of time, push cartswheelchairs, lift at least 50 pounds, to positionlift patients of various sizes and weights, visual and manual dexterity, provide CPR, etc.) to perform the essential functions of the position.
