Mom’s Meals is part of PurFoods LLC, a privately held and family owned/operated company based in Iowa.
The primary goal of this role is to increase awareness of HealthRelated Social Needs (HRSN) services in Oregon and promote Medically Tailored Meals (MTM) from Mom’s Meals across the state. HRSN Services are a part of the 1115 Demonstration Waiver that has allowed eligible Oregon Health Plan members to access services related to nutrition, housing, and climate. The ideal candidate will be able to clearly articulate the importance of nutrition in supporting positive health outcomes, especially for individuals managing chronic conditions, experiencing food insecurity, or facing other social determinant of healthrelated challenges. A strong passion for helping Medicaid beneficiaries access services that can improve their quality of life is essential.
This position requires the ability to build and maintain a wellorganized network of contacts and to identify organizations that serve eligible populations. The candidate will actively participate in webinars hosted by Coordinated Care Organizations (CCOs), county agencies, and statewide partners to stay informed on best practices, program updates, and future engagement opportunities. They will also frequently Mom’s Meals at inperson community events such as health fairs and resource fairs.
To drive MTM referrals, the candidate will educate organizations through phone calls, virtual meetings, and inperson networking. Success in this role depends on building strong, trusting relationships with a wide range of partners, including CommunityBased Organizations, housing and rental assistance providers, healthcare and behavioral health providers, Community Health Workers (CHWs), Federally Qualified Health Centers (FQHCs), clinics, hospitals and health systems, CCOs, outreach and engagement teams, and HRSN service providers.
Ideally, this position will be based in the Portland Area. It will also be eligible for a company car.

Position Responsibilities may include, but not limited to
Outreach to the list of referring groups above to engage and provide an overview of Moms Meals services and how to refer members to the program
Create and maintain target organization contact list
Ensure all contacts and locations for all target organizations are up to date in internal system
Offer and schedule onsite (preferred) or virtual inservices with target organizations
Be highly visible within the community by engaging in communitybased events
Attend and participate in county round tables and county and state webinars to represent Mom’s Meals
Visit Provider groups: Maternal Health practices, behavioral health, community transitions, local health clinics, FQHCs, WIC locations and others and provide overview of MM services
Stay up to date on referral workflows and Populations of Focus from HRSN Covered Populations and contracted health plans
Stay up to date with integration with closed loop referral systems and Mom’s Meals presence within systems
Stay up to date on Oregon Health Authority (OHA) and health planCCO reporting requirements and work internally to ensure reporting meets requirements
Required Skills and Experience
2 years of community health work
Demonstrated ability to successfully engage community health organizations
Proven ability to work with cross functional teams
Excellent oral and written communication skills; experience giving presentations
Experience using Microsoft Office suite, including PowerPoint and Excel
Willingness to travel regionally to events, up to 50% travel
Preferred Skills and Experience
Associates or Bachelor’s degree
Experience working with Medicaid or Medicaid Beneficiaries
Experience working with Behavioral Health Facilities, Housing and Rent Assistance organizations, or Food Security organizations
Study or work in social services or nutrition
