This is an on-site role that requires a regularly scheduled commute/travel to one of our locations in NYC, with a  willingness to cover additional boroughs like Manhattan and Brooklyn & Queens for field RN visits on occasion.

Department & Position Overview:

  • Deliver training programs and support clinical staff development to ensure competency and compliance. 

OVERVIEW

The LHCSA Nurse Educator is responsible for delivering FreedomCare's Personal Care Aide (PCA) training program, including 40-hour PCA courses, skills instruction, competency evaluation, testing, and required training documentation. The Nurse Educator also supports caregiver and clinical staff education, retraining, and competency development and performs field-based nursing work as assigned by the Director of Patient Services to support patient care, supervision, competency, and operational needs. 

KEY RESPONSIBILITIES

  • Conduct and facilitate 40-hour Personal Care Aide (PCA) training courses in accordance with NYSDOH requirements and the agency's approved curriculum, including classroom instruction, skills demonstrations, testing, and competency evaluation. 
  • Prepare for each class by ensuring required training materials, equipment, rosters, attendance records, examinations, skills checklists, and other program documentation are complete and available. 
  • Maintain complete and audit-ready training records, including enrollment, attendance, curriculum completion, examinations, competency evaluations, remediation, completion status, and required follow-up documentation. 
  • Provide remediation, retraining, and competency refreshers when a trainee or caregiver does not demonstrate required knowledge or skills, and document all follow-up actions. 
  • Collaborate with the Director of Patient Services, Recruitment/Certification, Compliance, and Learning & Development to support compliant PCA training, caregiver onboarding, and consistent education standards. 
  • Provide orientation, education, and competency support for new field nurses and retraining or clinical refreshers for existing nursing staff as needed. 
  • Perform field-based nursing work as assigned, including patient home visits, RN assessments or reassessments as appropriate, caregiver supervisory visits, competency assessments, clinical follow-up, patient/caregiver education, and other nursing activities within RN scope of practice and agency policy. 
  • Support audit readiness and quality improvement by maintaining accurate education records and ensuring training practices are applied consistently across LHCSA locations. 
  • Ensure trainees meet applicable program admission and completion requirements before training completion or certification documentation is finalized. 
  • Track class attendance, completion, testing results, competency needs, outstanding requirements, and follow-up actions; promptly escalate barriers that may impact certification, patient care, or compliance. 

CREDENTIALS

  • Current and unrestricted New York State Registered Professional Nurse (RN) license required. 
  • Associates Degree of Nursing Required 
  • Minimum of 1 year of experience in home care services (e.g., LHCSA, CHHA, hospice) required; home care education or supervisory experience preferred. 
  • Experience delivering healthcare training, caregiver education, competency evaluation, clinical onboarding, or adult education strongly preferred. 
  • Knowledge of PCA training requirements, adult-learning principles, caregiver scope of practice, infection control, patient rights, and clinical documentation standards. 

QUALIFICATIONS

  • Strong clinical judgment and ability to teach, evaluate, and reinforce safe care within RN and PCA scope of practice. 
  • Strong presentation, facilitation, written, and verbal communication skills with the ability to explain clinical and regulatory requirements clearly and professionally. 
  • Ability to engage adult learners, provide constructive feedback, and adapt instruction to different learning needs while maintaining required training standards. 
  • High attention to detail and ability to maintain accurate records in Salesforce, Medflyt, training platforms, portals, and other required systems. 
  • Ability to organize multiple classes, deadlines, field requests, and follow-up needs while escalating compliance or patient-care risks promptly. 
  • Commitment to patient rights, confidentiality, HIPAA, service quality, regulatory compliance, and FreedomCare values. 

WORK ENVIRONMENT

This position includes office, education-site, and field-based responsibilities. The Nurse Educator is expected to travel locally as assigned to support patient visits, RN assessments or reassessments as appropriate, caregiver competency assessments, supervisory visits, clinical follow-up, audit support, patient/caregiver education, or other nursing and operational needs. Field work may vary based on training schedules and agency needs. The role requires handling confidential patient, caregiver, and employment information in accordance with HIPAA and company policy.

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $90,000 and $105,000 per year at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
