Title: Independent Contractor – CPT-11084 Medical Scribe / Physician Documentation Assistant/Business Support/Advanced Level (Remote).

Overview

20four7VA is seeking offshore independent contractors to deliver remote services to clients worldwide, with a primary focus on developed markets such as the United States, the United Kingdom, Canada, and Australia.

We are seeking a highly experienced Medical Scribe / Physician Documentation Assistant to support a physician with clinical documentation, patient record management, and medical note preparation. A strong medical background is required, with preference given to licensed Medical Doctors due to the complexity and accuracy required in this role.

The ideal candidate must be able to accurately interpret clinical information, understand medical terminology, and efficiently create comprehensive patient progress notes within an Electronic Medical Record (EMR) system with minimal supervision. This role requires strong clinical judgment, speed, accuracy, and the ability to work in a fast-paced medical environment.

The objective of each engagement is to assist the client in improving business efficiency, enhancing service delivery, or supporting operational functions through high-quality, task-specific services rendered by a qualified contractor.

Scope of Services

Contractors will be required to provide services that align with the following client tasks:

Clinical Documentation Support

  • Assist the physician with daily patient documentation and progress notes.
  • Accurately transcribe and organize physician dictations.
  • Prepare structured and complete clinical notes for patient records.
  • Ensure all documentation is medically accurate and professionally formatted.

Patient Record Review & Summarization

  • Review and summarize patient concerns and clinical objectives.
  • Analyze vital signs, lab results, and diagnostic data.
  • Review nursing progress notes and incident reports.
  • Track specialist consults, follow-ups, and pending appointments.
  • Maintain clear and organized patient summaries for physician review.

EMR System Management

  • Navigate and update patient records within the PCC EMR system.
  • Ensure all entries are accurate, complete, and properly categorized.
  • Maintain structured documentation across patient charts.
  • Support ongoing EMR workflow efficiency and accuracy.

Real-Time Clinical Support

  • Assist the physician during rounding when real-time documentation is required.
  • Capture and organize clinical updates during patient interactions.
  • Support fast-paced decision-making with accurate documentation.

Compliance & Communication

  • Maintain strict HIPAA compliance and patient confidentiality at all times.
  • Collaborate with the physician through Microsoft Teams and other communication tools.
  • Ensure all communication and documentation follow medical and legal standards.

Each engagement is based on a defined scope of work and is governed by an independent contractor agreement. Services must be performed professionally, with adherence to any timelines, guidelines, and deliverables agreed upon in writing.


Skills

The Contractor must demonstrate proficiency in the following, depending on the Client’s requirements:

  • Strong clinical background with experience in medical documentation.
  • Deep understanding of medical terminology, patient care workflows, and clinical processes.
  • Experience using Electronic Medical Record (EMR) systems is required.
  • Ability to accurately interpret physician dictation with minimal supervision.
  • Ability to accurately update labs, medications, vitals, and dates of service.
  • Strong attention to detail and a high level of accuracy.
  • Ability to work quickly and efficiently while maintaining quality and productivity.
  • Excellent written English communication skills.
  • Ability to work independently, take ownership of assigned tasks, and manage responsibilities with minimal supervision.
  • Ability to work effectively in a high-pressure medical environment.
  • Prior experience as a Medical Scribe or Clinical Documentation Specialist is a strong advantage.
  • Familiarity with patient rounding workflows is preferred.

Note: The Company will make reasonable efforts to align the Assigned VA’s schedule with the Client’s preferred hours as set forth above. However, availability may vary based on time zones, capacity, and the contracted hours set in this SOW. Any changes must be mutually agreed to in writing through a revised SOW.

Terms of Engagement

  • Contractors will operate under a Master Services Agreement and a Statement of Work
  • This is not an offer of employment; contractors are not employees of 20four7VA or any client
  • All compensation is processed through 20four7VA according to the terms of the engagement
