Job description

The role of the Case Manager is to be an advocate for the members. The role operates in a team-like approach which provides support to new incoming case managers and existing ones. Targets are collective and each team member supports the other in achieving the overall Team's target. Compensation for Quarterly visits, weekly reporting and the opportunity to be a Team Lead your team, as Team lead is rotated within the team.


Some key responsibilities include:

Performing client/members oversight and management of home bound clients/members.

• Monitor health care plans for members enrolled in the EDWP Elderly and Disabled Waiver Program.

• Oversee service providers and ensure they comply with the organization's health care plan for each member.

• Propose changes to the health care plan as needed to help member(s) improve or stabilize their health condition(s);

• Coordinate the flow of information between members and service providers;

• Establish and oversee efficient work relations with all stakeholders;

• Assist members make informed decisions by acting as their advocate for matters specific to their clinical status and treatment options;

• Contact members by phone monthly and as needed to evaluate their health condition and needs;

• Physically visit members on a quarterly basis and as needed to evaluate their health condition and needs; post COVID-19

• Participate in weekly meetings that aim to discuss and resolve member issues and facilitate member needs;

• Adhere to the professional standards as outlined by the protocols, rules and regulations stipulated by the organization and the Department of Community Health.

  • Must have flexible work schedule to accommodate meetings that is set and also Ad hoc by the organization.


Bi-Lingual a plus: Vietnamese a plus!

Other Languages such as Spanish, Korean,. Vietnamese, Russian, would be a plus. etc.


Skills needed: Computer literacy, goal oriented, Multi- tasking, flexibility, Adaptability to change, able to work independently.
