Triage on-call every other week 7 on 7 off 5pm-8:30am

About the Role:

The RN Triage Nurse at the TX - Our Tyler, Longview and Sulphur Springs Hospice Branches plays a critical role in providing timely and accurate clinical assessments to patients and their families, ensuring appropriate care pathways are followed. This position involves evaluating patient symptoms, prioritizing care needs, and coordinating with interdisciplinary teams to facilitate effective hospice care delivery. The nurse will serve as a vital communication link between patients, families, and healthcare providers, offering guidance and support during emotionally challenging times. By utilizing clinical expertise and compassionate communication, the RN Triage Nurse helps to optimize patient comfort and quality of life. Ultimately, this role contributes significantly to the overall mission of delivering exceptional hospice care tailored to individual patient needs.

Minimum Qualifications:

  • Current and unrestricted Registered Nurse (RN) license in the state of Texas.
  • Minimum of 2 years clinical nursing experience, preferably in hospice, palliative care, or emergency nursing.
  • Strong knowledge of triage principles, symptom assessment, and patient prioritization.
  • Proficiency in electronic health record (EHR) documentation and clinical communication.
  • Excellent verbal and written communication skills with the ability to provide compassionate patient and family support.

As a Hospice RN, you will:

  • Coordinates care across disciplines to ensure the provision of quality care. 

  • Maintains patient confidentiality. 

  • Ensures timely communication to all appropriate case team members of documentation.

  • Assess communications, orders and rehabilitation measures from Clinical Mangers to ensure individual needs are met on a pt by pt bases.

  • Adhere to plan of care on all assigned patients to provide optimal care. 

  • Created and implement and changes to the patients medical record as needed and agreed upon by the Clinical Manager, DCS and physician. 

  • Perform lead functions and communications related to integration and coordinator of care e.g. facility staff, physicians, pharmacy, case team, supervisor, etc.

  • Attend staff meetings and in-services.

  • Perform nursing duties as scheduled and assigned.

  • Provide accurate, timely, complete and acceptable documentation.
