We support our team with a competitive compensation and benefits package, including:
- Annual compensation between $115,000-$130,000, PLUS an annual performance-based, discretionary incentive.

What You Will Do - Essential Functions
Please note that although this is a full-time telehealth position, candidates must hold an active, unrestricted New York State clinical license as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Clinical Care Delivery & Care Coordination
- Deliver comprehensive virtual care, including patient assessments, condition management, medication review, and appropriate escalation, while ensuring compliance with telehealth regulations and licensure requirements.
- Collaborate with multidisciplinary teams and utilize telehealth workflows to support coordinated, continuous care and maintain accurate documentation of patient encounters.
- Patient Engagement & Education
- Educate patients on preventive care, disease management, and healthy lifestyle behaviors to improve outcomes and encourage active participation in their health.
- Provide compassionate, culturally sensitive care while maintaining professionalism, strong communication, and sound clinical judgment in all interactions.
- Quality & Compliance
- Adhere to clinical guidelines, regulatory standards, and organizational policies while participating in quality improvement initiatives.
- Ensure accurate, timely documentation, uphold patient privacy, and maintain high standards of safety, ethics, and confidentiality.
Who You Are
Required Qualifications:
- Active, unrestricted current NY licensure as a Provider in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions.
- NY State only: Minimum of 3600 hours of practice experience, or with 3 years of clinical experience (autonomous provider), in a primary care or longitudinal care setting
- Board Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant
- Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements.
- Active Drug Enforcement Administration (DEA) license.
- Current Basic Life Support Certification for health care providers.
- Proficiency in electronic medical record (EMR) systems.
- Active, unrestricted current licensure as a Provider in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions.
- Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant
- Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements. Preferred two years of experience in a primary care or longitudinal care setting.
- Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.

- Have a strong understanding of risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA).
- Ability to document assessment findings accurately and thoroughly
- Maintain patient, team member and employer confidentiality while remaining in compliance with all HIPAA regulations.
- Ability to make critical decisions while following Lucet policies and procedures.
Working conditions:
- Work is performed from home with company-provided equipment. Sitting for long periods of time is expected and use of fingers and hands for typing is necessary.
- A quiet workspace with minimal background noise for calls.
- High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting).
