Job Description:

Reviews electronic medical records to validate the appropriateness of medical records and performs electronic corrections when integrity issues are identified. Analyzes chart correction scenarios and performs hands-on technical chart corrections. Maintains the Enterprise Master Patient Index (EMPI) to ensure accurate patient information across all systems. Collaborates with multiple departments enterprise-wide to coordinate and correct patient information and other required data. Ensures timely correction of inappropriate data in the patient's chart throughout all inpatient and outpatient information systems. Utilizes various systems to research, analyze, and validate patient information. Compares key attributes within the patient's health record to ensure accuracy. Processes document correction requests in a timely manner for all facilities. Accurately documents information pertaining to chart correction cases, including entering case notes, root causes, advisories, and assigning case tasks as needed. Reviews and validates incoming messages from other facilities to determine if they should be linked to an existing patient or rejected. Collaborates with teams at corporate and facility levels to ensure reliable and trustworthy health records. Ensures health records are free from erroneous data or unauthorized modifications. Other duties as assigned.

Knowledge, Skills, and Abilities:
• Exhibits excellence in communication, feedback, exactitude, and follow through with data, customers, and peers, as there is a high level of patient safety risk associated with the transfer of patient information. [Required]
• Strong analytical problem solving skills operating with a high level of accuracy [Required]
• Strong ability to work independently, demonstrating effective time management and delivering results without constant supervison. [Required]
• Excellent time management skills and the ability to multi-task [Required]
• Knowledge of Microsoft Office, i.e. excel, word, outlook [Required]
• Knowledge of medical treatment methods, pharmacology, patient care assessment, medical documentation requirements, and data collection techniques. [Preferred]

Education:
• Associate [Preferred]

Field of Study:
• in Health Information Management or related field

Work Experience:
• 2+ of health information management experience [Required]
• Experience using EPIC as it pertains to Chart Corrections or Health Information Management [Preferred]
• Previous experience in Health Information Management or related healthcare field. [Required]
• Prior experience with other EHR systems i.e., Cerner, Athena, & Initiate [Preferred]

• Registered Health Information Administrator (RHIA) [Preferred]
• Registered Health Information Technician (RHIT) [Preferred]
