Overview:

Sentara Home Care is seeking to hire an LPN Intake Coordinator.

Overview

Evaluating and processing referrals by receiving and responding to incoming calls from referral sources or potential patients. Record the outcome of calls and make follow up calls as necessary. Enter referral information in system including clinical information and orders for services. Develop and maintain a working knowledge of all services and resources provided by Sentara and available within the community. Act as a resource for patient and referral source issues.

Anticipated Status/Shift:

Flexi (15 hours/week)

Day Shift, Weekend hours as needed

Position Location:

Remote with occasional onsite meetings and trainings in Chesapeake, VA. Eligible candidate must live no more than one hour away from Chesapeake, VA.

Hourly Range: $17.31 - $28.80/hour

Education

• HS Diploma or Equivalent (Required)

• Trade School Graduate (Preferred)

Certification/Licensure

• LPN License - Active in Virginia (Preferred)

Experience

• 1-year of experience with clinical and customer service in a healthcare environment (Preferred)

• Care coordination experience (Preferred)

• Home Health experience (Preferred)
