  • Support the design and delivery of culturally relevant interventions, communications, and services with member/community input; ensure regulatory reporting, quality improvement, and adherence to state/federal requirements; travel as needed.

The Senior Population Health Strategy Professional is a key leader supporting the strategic direction and operational execution of population health initiatives with a primary focus on chronic disease prevention, intervention, and management within the Oklahoma Medicaid market. This role leverages clinical expertise to drive evidence-based strategies that identify, address, and reduce health disparities, improve health outcomes for members with chronic conditions, and ensure cost effective care.

The Senior Population Health Strategy Professional uses independent judgement requiring analysis of variable factors and determining the best course of action and works on problems of diverse scope and complexity ranging from moderate to substantial under minimal supervision.

Position Responsibilities

  • Lead the planning, coordination, and execution of high-impact population health initiatives centered on chronic disease (e.g., diabetes, cardiovascular disease, asthma), ensuring alignment with organizational and state Medicaid priorities and requirements. 

  • Apply data-driven approaches to identify health disparities and population specific needs among Medicaid populations  

  • Collaborate with internal and external partners to advance equitable care and address social determinants of health as they relate to chronic disease outcomes. 

  • Develop and disseminate actionable reports, translating data into insights that inform strategies, guide strategic resource allocations, partnership development and support continuous quality improvement.  

  • Support the design and delivery of culturally relevant interventions, communications, and services, incorporating member and community input to ensure appropriateness and effectiveness. 

  • Manage multiple projects and partnerships simultaneously, maintaining strong relationships with key stakeholders and ensuring timely, compliant, and impactful execution. 

  • Support regulatory reporting and documentation ensuring adherence to state and federal requirements, with emphasis on outcome measurement. 

  • Champion organizational values that respect and engage diverse communities. 

  • Regularly travel throughout the Oklahoma market for events and meetings. 

  • Demonstrate strong facilitation, communication, and consensus building skills

Required Qualifications

  • Bachelor's degree in Nursing, Public Health, Health Policy, other clinical/science majors, or related field. 

  • Live in Oklahoma 

  • Minimum five (5) years of progressively responsible experience and leadership in population health management, care management, nursing, chronic disease management, or related fields. 

  • Demonstrated experience working directly with Medicaid population. 

  • Experience developing and executing population health and chronic disease initiatives and quality improvement strategies. 

  • Ability to effectively communicate with clinicians, members, and community partners regarding chronic disease management strategies. 

  • Proven ability to analyze and interpret data, make data-driven recommendations, and translate findings to multiple stakeholders 

  • Proficiency in Microsoft Office applications (Word, Excel, PowerPoint), familiarity with data analytics tools like PowerBI dashboard, etc. 

Additional information 

  • This is a remote position with regular travel (up to 25%) required within the Medicaid market for meetings, events, and community engagement activities. 

  • Must have a valid driver’s license and reliable transportation. 

  • Work-from-home requirements include a dedicated, interruption-free workspace and high-speed internet connectivity. 

Preferred qualifications 

  • Experience in healthcare, payor clinical programs, clinical leadership, medical setting, and/or value-based care organizations 

  • Master's degree or clinical licensure: preferably in nursing (e.g., RN, LPN), public health, healthcare administration, other clinical/science majors, related fields, or other allied health professional

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$78,400 - $107,800 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
