What We’re Looking For
Trillium Health Resources has a career opening for a Support Transition and Reentry (STAR) Senior Manager to join our team! The STAR Senior Manager provides leadership, oversight and supervision for a multidisciplinary team delivering NC Medicaid Tailored Care Management (TCM) services to justice-involved members with behavioral health (BH), intellectual/developmental disabilities (I/DD), and/or substance use (SU) needs. The STAR program assists adults exiting a Department of Corrections or County Detention facility who also experience BH or IDD conditions. This role ensures the delivery of whole-person, integrated care management in alignment with NC Medicaid Tailored Plan requirements, including comprehensive assessment, person-centered care planning, care coordination, transition of care, and health promotion.
On a typical day, you might:
- Train Coordinators and Health Specialists on systems and processes needed to assist with successful Community re-entry.
- Build from the resources of the community to assist in linking to services that meet the needs of the members and families within their homes and communities
- Oversee and support staff who coordinate and manage care for members who are participants of the Transitions to Community Living program.
- Lead and support audit readiness and response activities, including NC Medicaid, NCQA, and EQR reviews, ensuring full compliance with Tailored Care Management documentation, service delivery, and performance standards.
- Ensure all care management activities are delivered in a manner that is person-centered, trauma-informed, culturally competent, and compliant with NC Medicaid Tailored Plan requirements, with a focus on improving health outcomes, advancing health equity, and supporting successful community reentry.
- Ensure staff are engaging members in the transition process with reflective listening and motivational interviewing.

Qualifications
Required:
- Associate’s degree with four (4) years of experience working with behavioral/mental health, substance use and/or intellectual/developmental disability populations;
OR
- Bachelor’s degree with two (2) years of experience working with behavioral/mental health, substance use and/or intellectual/developmental disability populations;
OR
- Equivalent combination of education/experience.
- Must have a valid driver’s license
- Must reside within 75 miles of Trillium’s Catchment of North Carolina. Counties included: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson.
- Must be able to travel within catchment as required.
Preferred:
- Human Services or Criminal Justice degree.
- Master’s degree
- Minimum of one (1) year working in criminal justice or juvenile justice systems.
- Minimum of two (2) years’ experience supervising a team (2 or more employees)
- Education, training, and Work experience that shows a history of learning and applying the skills needed to work with a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diverse backgrounds.
