Roles & Responsibilities

  • Must meet Qualified Professional (QP) status or be fully licensed by the NC governing board regulating Human Services professions; OR be a licensed RN licensed in NC with four or more years of MH/DD/SA experience.
  • Alternative pathways include: Master's degree in a human service field with at least 1 year MH/DD/SA experience; or Bachelor's degree in a human service field with at least 2 years MH/DD/SA experience; or Bachelor's degree in another field with at least 4 years MH/DD/SA experience (with applicable licensure/certification as required).
  • Minimum two years of LTSS/HCBS coordination or care delivery monitoring experience if serving members with LTSS needs.
  • Valid driver's license and residence within 75 miles of Trillium’s NC catchment; ability to travel within the catchment as required.

Requirements:

  • Provide complex care coordination and monitoring for members with primary IDD, including potential co-occurring mental health or substance use needs, ensuring adherence to waiver requirements and integration with multidisciplinary teams.
  • Conduct discovery activities and use person-centered planning to gather information, understand member needs, and develop responsive care plans.
  • Educate and support members on rights and responsibilities, and coordinate care across providers within a multidisciplinary framework.
  • Update assessments and care plans as members’ life circumstances change and ensure ongoing alignment with waiver requirements and quality standards.

Posting Salary Range: $54,106 - $67,210

Office Location: Remote within 75 miles of Trillium’s Catchment of North Carolina. (See requirements section for included counties)

What We’re Looking For


Intellectual/Developmental Disability (I/DD) Care Manager – 1915i is responsible for providing complex care coordination and monitoring to members having a primary Intellectual or Developmental Disability, which may include a secondary mental health or substance abuse, and/or complex medical/physical health need. This position requires a dynamic, proactive approach to assessment, monitoring and complex coordination of care, to ensure quality support and consistent adherence to waiver requirements. The I/DD Care Manager assists with integrated care management through multi-disciplinary collaboration.


On a typical day, you might:


  • Utilize person-centered planning methods & strategies to gather information, to get to know the members supported and to adequately address specific needs of the individual.
  • Provide education and support to members on learning about and exercising rights and responsibilities. 
  • Complete discovery activities (information gathering and assessment) as a component of the planning process.
  • Ensure that assessments and plans are updated as needed and whenever the member’s life circumstances change.

Qualifications


Required:

  • Must meet QP status. 
  • Fully licensed by the NC governing board regulating Human Services professions; OR 
  • Fully licensed RN who is licensed to practice in the state of NC by the NC Board of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served. OR 
  • A graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, pre or post-graduate degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has one year of full-time, pre or postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR 
  • A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has two years of full-time, pre or post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR 
  • A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has four years of full-time, pre or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling. 
  • Additionally, If serving members with LTSS needs, the care manager, must meet the minimum QP requirements defined above (A-D) and shall additionally have at a minimum two (2) years of prior LTSS and/or HCBS coordination, care delivery monitoring and care management experience. This experience may be concurrent with the years of experience required to become a Qualified Professional.
  • License and/or certification may be accepted as noted above.
  • Must have a valid driver’s license.
  • Must reside within 75 miles of Trillium’s Catchment of North Carolina. Counties included: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson.
  • Must be able to travel within catchment as required.
