  • Degree in Health and Human Services or related field (e.g., Psychology, Social Work, Nursing, Counseling) from an accredited institution.
  • Certification/Licensure: none required.
  • Minimum 1 year of experience in Long-Term Care or Health Plan/Medicaid; Medicaid experience preferred.
  • Case management or mental health experience; SMI/SED/SUD experience preferred.

Requirements:

  • Perform casework duties and provide case management services to patients, families, and designated caregivers.
  • Develop, participate in, and monitor multidisciplinary collaboration of services for patients where appropriate.
  • Assist adult patients and their families with personal and environmental difficulties related to medical conditions, including end-of-life considerations.
  • Conduct in-person assessments (1–3 times per week) and remote telephonic assessments for adult and pediatric members with SMI, SED, and SUD across Virginia Beach, Norfolk, Chesapeake, Portsmouth, Suffolk, and the Eastern Shore.

Overview:

Sentara Health Plans is hiring a Care Coordinator for SMI/ SED/ SUD in Virginia Beach, Norfolk, Chesapeake, Portsmouth, Suffolk, and the Eastern Shore, VA!

Status: Full-time, permanent position (40 hours)

Standard working hours: 8am to 5pm EST, M-F

The position requires both in‑person, face‑to‑face assessments (1–3 times per week) and remote telephonic assessments of adult and pediatric members with Serious Mental Illness (SMI), Severe Emotional Disturbance (SED), and Substance Use Disorders (SUD) throughout Virginia Beach, Norfolk, Chesapeake, Portsmouth, Suffolk, and the Eastern Shore, as needed.

Location: Candidate must reside in Virginia Beach, Norfolk, Chesapeake, Portsmouth and Suffolk, VA.

Job responsibilities:

Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.

Schools that are not listed on the database do not meet the standard as accredited. Degrees that have been obtained from schools outside the United States will be reviewed individually.

Certification/Licensure

  • None required

Experience

  • Long Term Care- 1 year REQUIRED

  • Health Plan- 1 year; Medicaid- preferred

  • At least one year of mental health experience preferred

  • Case Management experience preferred

  • Experience in Serious Mental Illness (SMI), Severe Emotional Disturbance (SED) and Substance Use Disorder (SUD) preferred
